Mount Sinai South Nassau provides a salary range in good faith determination of potential compensation to comply with the New York State law on Salary Transparency in Job Advertisements. Actual salaries depend on a variety of factors, including experience, education, and hospital need. Position Summary

Summary:
The Coordinator serves as the liaison between Mount Sinai South Nassau and the TCU; choosing appropriate patients to be admitted to TCU who will benefit from a short term sub- acute admission. The coordinator is responsible for planning, organizing, implementing and evaluating programs for the TCU admission process. He/she will direct all activities toward the fulfillment of the philosophy, goals and objectives of the facility. He/she will also coordinate the admission assessment of patients to comply with Centers for Medicare & Medicaid Services (CMS) and the Department of Health requirements, assessing patients for Medicare and managed care benefits. CCAR is responsible for the planning, organizing, implementation, and evaluation of MDS Medicare Assessments, Care Planning requirements, interdisciplinary team coordination of care and CMS regulations pertaining to such to maximize reimbursement.

Qualifications

Responsibilities:
Education Required
Must complete in-house orientation program
Graduate of an accredited program for Registered Professional Nursing

Experience And Skills
Excellent interpersonal skills
Speaks, reads and writes English
Minimum 3 years clinical experience, including Long Term Care (LTC) experience required.
3-5 years experience with MDS assessment.

License/Certifications
Current and valid New York State registration authorizing practice as a Registered Professional Nurse (RN)
BLS
Certification in MDS preferred, not required.
