Job Description
SUMMARY OF DUTIES AND RESPONSIBILITES
Minimum Qualifications
1. Valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Education Department (NYSED); and
2. A Baccalaureate degree in Nursing or related health field from an accredited college or university; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and
4. Two (2) years of experience as a Registered Professional Nurse.

Job Description
SUMMARY OF DUTIES AND RESPONSIBILITES
- Screens patients and actively identifies high-risk individuals and refers them for services
- Provides education to high-risk patients on lung cancer screening options.
- Facilitates access to screening appointments
- Assists patients with navigating the screening process.
- Assists with the identification of potential barriers to care including but not limited to financial, emotional, psychological and physical barriers.
- Facilitates access to care by collaborating with patients, their support system and members of the interdisciplinary team in addressing identified or perceived barriers.
- Collaborates with the multidisciplinary health care team to optimize lung cancer screening.
- Provides support throughout the screening journey.
- Engages with patients and promotes adherence to treatment regime including maintaining doctor's appointments.
- Launches education and patient engagement initiatives to maximize the impact of cancer screening.
- Educates staff and promotes staff awareness of the lung cancer screening program and initiatives.
- Discusses and shares smoking cessation literature and techniques with patients.
- Acts as facility liaison for Woodhull with Central Office and other system sites.
- In collaboration with the interdisciplinary team collects, reviews quality indicators and metrics relating to the lung cancer screening program.
- Prepares reports and presentations for Quality Departmental and Central Office meetings.
- Develops a tracking system for lung cancer screening patients and coordinates with multiple services to ensure appropriate follow- up.
- Maintains appropriate documentation in patients' medical record.
- Performs other duties as required including wound care, post-operative procedure evaluation and teaching.
