Addiction Medicine Physician
Monday-Friday 8:00am - 5:00pm (no weekends, nights, or major holidays)
Full-time, 40/hour per week

Key Responsibilities:
- Collaborates with other members of the care team to provide high quality SUD care and primary care to CHS patients.
- Conduct thorough assessments of patients, including medical, psychological, and substance use history.
- Develop individualized treatment plans based on patients' needs, including pharmacological and therapeutic interventions.
- Administer or supervise medication assisted treatments (MAT).
- Educate patients and their families about the nature of addiction, treatment options, and recovery strategies.
Qualifications:
- FNP, PA or MD/DO.
- Active state licensure and certification.
- Experience and passion for providing addiction medicine care to underserved patients preferred.
