Job Overview:
The Advanced Practice Provider (APP) in Nephrology works under the supervision and collaboration of a qualified physician to deliver exceptional care to patients in both hospital and office settings.
Responsibilities:
- Conduct weekly rounds on dialysis patients across multiple facilities, including gathering detailed medical histories, performing comprehensive physical exams, diagnosing medical issues, ordering and interpreting diagnostic imaging, and initiating appropriate treatment plans.
- Order medications and manage prior authorizations as needed.
- Document patient visits, transition of care visits, and procedure clearance appointments accurately.
- Participate in monthly interdisciplinary patient care meetings with social work, dieticians, and charge nurses.
- Collaborate closely with the supervising physician to ensure at least 4 visits per month and maintain accurate billing and documentation records.
- Manage telephone calls from hospital units and patients, addressing inquiries and providing necessary information.
- Coordinate acute hospital admissions with the covering hospital nephrologist.
- Work with a patient population that includes in-center hemodialysis, home hemodialysis, and peritoneal dialysis patients.
Education requirements:
- NP - Current RN license and current AANP or ANCC certification
Certification:
- BLS/ACLS/AED training required and accepted from only 2 organizations:
- American Heart Association and American Red Cross - BLS/ACLS for Healthcare Providers (CPR & AED) Program
- No online programs from any organization will be accepted.
