Job Description:
The BMT Coordinator is responsible for assessing the needs, education, and coordination of care for BMT and certain blood cancer patients and their families throughout the disease process. The Coordinator communicates all aspects of the plan of care with patients, families, the Transplant Team, other healthcare providers, and payors.
Responsibilities:
- Monitor patient/family before, during, and after transplantation for physical, emotional, psychosocial, and financial needs, intervening when appropriate.
- Refer patients to members of the BMT team (pharmacist, social work, nutritionist, oncology support team, and financial advisors) and/or utilize community resources when appropriate.
- Coordinate patient pre-admission work-up and visits, gather and review all pertinent clinical information, and present this to the transplant physician, highlighting any abnormal results.
- Collaborate with direct healthcare providers to establish a plan of care, ensuring continuity of care across departments (in-patient and out-patient) and between hospital and home health providers.
- Coordinate all services for BMT and targeted blood cancer patients and oversee necessary processes as needed.
- Manage the transfer of patient care back to the referring physician and oversee long-term follow-up, including periodic updates on patient condition, coordinating follow-up visits, and coordinating post-discharge services.
- Educate patients and families to help them understand the disease, processes, and procedures surrounding pre- and post-transplant issues.
- Act as a resource for the interdisciplinary team on transplant care and protocols to promote positive outcomes for BMT patients.
- Communicate with the interdisciplinary team, patients, families, and payors in a timely manner to ensure comprehensive documentation.
- Perform admissions and continued stay reviews as needed.
- Act as an advocate for patient benefits and manage outpatient drug benefits, including investigating formulary issues and obtaining overrides.
- Identify the need for indigent drug programs and coordinate the application process.
- Stay current on requirements mandated by accrediting and regulatory bodies, monitoring compliance.
- Support the mission and vision of the organization, demonstrating excellent customer service at all times.
- Participate in weekly transplant team meetings, preparing and presenting information as needed.
- Enter select patient data into databases, reviewing and updating information as required.
- Attend meetings and participate in committees as appropriate.
Required Qualifications:
- Graduation from an accredited nursing program
- Relevant Registered Nurse License
- National Provider BLS Certification
- 1 year of Hematology Oncology Nursing experience
Preferred Qualifications:
- Oncology Certified Nurse (OCN)
