Ambulatory Care Pharmacists work to provide optimal, safe, and evidence-based medication therapy to patients of all ages, backgrounds, and needs in outpatient settings throughout multiple health-system campuses. These clinicians provide direct patient care services that include but are not limited to chronic disease management, medication education and counseling, medication management, medication reconciliation, and medication access. Advanced practice activities are performed pursuant to collaborative practice agreements and protocols, such as prescribing and/or administering medications, ordering and interpreting labs, performing point of care testing, and modifying therapy plans according to patient response.

- Current Pharmacist licensure in good standing in the State of Ohio.
- Graduated from an accredited School of Pharmacy.
- Doctor of Pharmacy degree or Bachelor of Science degree or equivalent.
- Certifications required: BLS, Administration of Immunizations (Ohio Board of Pharmacy compliant course).
