Harlem Hospital Center has been a bedrock of the Harlem community since 1887. The hospital provides a wide range of medical, surgical, diagnostic, therapeutic and family support services to the residents of Central Harlem, West Harlem, Washington Heights and Inwood. Harlem is the largest hospital in the area, capable of treating the most seriously ill. Harlem provides over 90 specialized ambulatory care services, dentistry and oral surgery, behavioral health services, and community substance abuse treatment.

Duties & Responsibilities
Under general supervision of the Director of Nursing, assists with the planning, organizing, directing, coordinating and evaluation of nursing service and educational programs within the transitional care division of Care Management. The expectation is to work together with the transitional care team to reduce 30-day readmissions and serve the high-risk population of patients post discharge from the inpatient med/surg units as well as the Adult Emergency Department to safely return to the community setting. Organize and provide oversight of the care-coordination necessary to optimize the individual's optimal health in the community.
Essential Duties and Responsibilities:
Performs all duties of the Director of Nursing in his or her absence.
Participates in developing policies and standards of nursing service and education in the area of care management - transitional care.
Assumes responsibility for developing and implementing a staffing plan and directing recruitment and selection of personnel.
Assumes responsibility for planning, directing, organizing and evaluating the compass rose transitional care pilot to its maturity.
Assumes responsibility of the day-to-day operations of the transitional care division of care management. Oversee the TCM program at Harlem Hospital and report to the Director
Provides oversight and direction to the Accountable Care Managers (RN's) and Care Coordinators (CLW's) as well as clerical and ancillary staff to meet the goals of the transitional care division.
Plans and implements strategies to reduce 30-day readmissions and ED Visits to achieve positive client/patient outcomes.
Able to respond to Performance data by utilizing the PDCA cycle (Plan Do Check Act)
Maintains effective communication with physicians, colleagues, clients/patients, families and others related to post discharge plan of care.
Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse; and
2. Master's degree from an accredited college or university in Nursing Administration or a related field and four (4) years of high-level responsible experience in nursing administration, three (3) years of which must have been in a hospital; or
3. A satisfactory equivalent combination of education, training and experience. However, all candidates must be licensed and currently registered to practice as a Registered Professional Nurse in New York State.
