Major Responsibilities:
  • Performs patient psychosocial assessments to determine appropriate intervention and care plan, goal determination and treatment planning altering same as case warrants. Contacts families, where appropriate, for collateral information and discharge planning. Facilitates family meetings, communicates schedule and outcomes with members of the interdisciplinary team.
  • Functions as a member of the interdisciplinary treatment team by reviewing assessment data, case management, intake and other discipline data. Ensures goals identified in the treatment plan are clinically appropriate to the stated problems, developmentally focused, measurable and achievable within current anticipated lengths of stay, and updates treatment plans as appropriate.
  • Collaborates and coordinates efforts with treatment team members to complete the discharge plan in a timely manner.
  • Attends and participates in treatment teams and meetings and may be assigned to lead and coordinate meetings as needed. Facilitates communications between and negotiates with the patient/family, physicians, nurses, dietary, rehab, homecare, social services, and other disciplines that need to collaborate to provide care for the patient.
  • Communicates effectively with patients, family, treatment team and community agencies and facilitates resolution of issues which could impact continuality of care. Works cooperatively with other professional and non-professional staff to identify and resolve problems in the service delivery systems, ultimately improving the quality of patient care.
  • Responsible for the discharge needs of all patients and the transition of all patients to the next level of care, ensuring quality across the continuum of care. Educates patient on discharge process while focusing on a positive discharge experience to the next level of care. Coordinates aftercare services including referrals for housing, case management, transportation services, meal services, peer support groups, and vocational referrals.
  • Meets with patients that have been readmitted within 30 days to discuss barriers to treatments. Discusses treatment options with patient and treatment team out ensure positive treatment outcomes upon discharge.
  • Accurately completes all documentation responsibilities as outlined in department/service area procedures within established timelines. Documentation responsibilities include but are not limited to: treatment plans, progress notes, discharge summaries, aftercare plans, records for billing, etc.
  • Acts as a liaison with referral sources to ensure continuity of care and discharge planning. Facilitates timely and appropriate referrals to other community resource networks such as outpatient providers of mental health and AODA services, home health agencies, residential treatment facilities and other community agencies specializing in financial, housing, child care and transportation services. Documents these activities appropriately.

License and Certifications:
  • Professional Counselor license issued by the state of Wisconsin, or Marriage and Family Therapist license issued by the state of Wisconsin, or Social Worker, Clinical license issued by the state of Wisconsin.

Education:
  • Masters Degree in Social Work, or Master's Degree in Psychology, Clinical, or Master's Degree in Psychology, Educational, or Master's Degree in Psychology, Counseling or related field.


Experience:
  • 2 years experience of intake and inpatient setting.


Knowledge, Skills & Abilities
  • Good written and verbal communication skills.
  • Demonstrates the ability to function in a complex mental health delivery system.


Physical Requirements and Working Conditions:
  • Will frequently be required to lift up to 25 lbs. without assistance and will occasionally be required to lift over 25 lbs. with assistance.
  • May be exposed to the following hazard: mechanical, electrical, chemical, blood and body fluids.
  • Must be able to sit, stand, walk, squat, bend, and reach above shoulders and twist.
  • Must be able to restrain and control patients who are outwardly psychotic or physically combative.
  • Operates all equipment necessary to perform the job.

Pay Range
$32.45 - $48.70
