SALARY RANGE: 67,891.20 - 110,947.20YEARLY
General Summary:
The Behavioral Health Clinician (BHC) will evaluate and treat patients for mental, emotional, and behavioral disorders. They will primarily provide patients with one-on-one short-term interventions, including psychotherapy, education, motivational interviewing, skill training, and CBT interventions. The BHC will work collaboratively with primary care clinicians in the office environment through warm handoffs, behavioral treatment recommendations, trainings, and program development. BHCs will assist with both acute needs as well as supporting and/or referring patients requiring long-term treatment for appropriate follow-up care. Under the BHC model, approximately 40% of time is spent in scheduled therapy sessions and 60% spent available for warm handoffs at the point of care. This role requires an advanced degree and license to practice. Prior experience in an integrated primary care behavioral health model is preferred.
Responsibilities:
1. Clinical services in the outpatient setting including: Diagnostic assessment, Short-term individual and group psychotherapy and counseling, Couples and family therapy if competencies present in this area, Crisis assessment and intervention, Brief psychoeducation and intervention as part of integrated care visits, Health and behavior coaching in individual and group formats, Utilize telehealth capabilities, when appropriate.
2. Work as part of an interprofessional team in a patient-centered clinical environment
3. Work with population health leadership and clinicians to proactively identify patients who would benefit from health psychology focused (e.g. hypertension, diabetes management, weight loss, tobacco use) interventions.
4. Consult with PCPs in real-time to enhance understanding of the patient, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans;
5. Close all charts within twenty-four (24) hours of visit - ability to do concurrent/concordant charting/typing; including assessments, progress notes, and billing;
6. Attend on-site/off-site community engagement activities and on-site/off-site clinic events as needed.
7. Assist with scheduling of patients for therapy.
8. Attend practice meetings.
9. Service on other committees and administrative assignments as needed.
10. All other duties as assigned within the scope and range of job responsibilities.

Required Education, Credential(s) and Experience:
- Education: Master Degree Counseling
- Credential(s): Licensed Clinical Social Worker; Or licensure as a marriage and family therapist, or related discipline in the state of Delaware.
- Experience: Required: None.
Preferred: Two years' experience providing psychological assessment and interventions to individuals and families across the lifespan. Experience in providing integrative behavioral health care with a team of primary care clinicians and working with teams of health care providers and support staff from different disciplines. Experience providing psychotherapy to a socioeconomically, racially, and ethnically diverse population.
