Job Overview
The CHF Program Manager is responsible for managing, implementing, and evaluating the clinical operations related to heart failure within the cardiovascular service line. This person works with a multidisciplinary team to provide improved comprehensive cardiovascular services to patients. This position is responsible for the efficient and effective management of Heart Failure services and the development and management of Heart Failure services across the service line. Holds self and staff accountable for identification, evaluation, documentation, and appropriate intervention of patients' needs to ensure quality and cost-effective patient care. Develops and maintains systems to assure efficient use of resources, both human and material. Serves as the key liaison to physician in areas assigned. Ability to integrate all areas of heart failure care within the continuum of patient needs regarding their chronic illnesses to allow for smooth transitions between the different levels of care needed.
Required: Certifications/Licenses/Education
Must have a current, applicable state RN license with at least five years of experience in the Cardiovascular field.
Experience with CHF Programming or experience in the care of the CHF patient population is highly desirable.
Must be a graduate of an accredited school of nursing as a Registered Nurse.
Bachelor of Science in Nursing (BSN) required, MSN, MHA, or MBA preferred.
