Key Responsibilities:
- Lead a team of care managers, and support staff, providing guidance, training, and performance evaluations.
- Oversee the daily operations of the care management department, ensuring compliance with organizational policies and regulations.
- Coordinate staffing and scheduling to ensure adequate coverage for patient care needs.
- Develop, implement, and monitor patient care plans to ensure they meet the needs of patients, improve health outcomes, and reduce hospital readmissions.
- Collaborate with multidisciplinary teams (e.g., doctors, social workers) to develop individualized care plans for patients with complex medical needs.
- Review patient data to assess risks, evaluate progress, and adjust care plans as necessary.
- Create and implement strategies to improve care management processes, focusing on patient-centered care and reducing unnecessary healthcare costs.
- Monitor performance metrics (e.g., patient satisfaction, readmission rates) and adjust care management strategies based on these outcomes.
- Lead initiatives to enhance care transitions, discharge planning, and chronic disease management.
- Ensure that all care management practices comply with healthcare regulations, accreditation standards, and payer requirements.
- Oversee documentation and reporting to ensure accurate and timely submission of data for audits and compliance.
- Establish and maintain partnerships with community resources, and other external service providers to enhance care coordination.
- Work closely with insurance companies to ensure patients receive necessary services while managing costs effectively.
- Manage departmental budgets, ensuring efficient use of resources.
- Identify opportunities to reduce costs while maintaining high-quality patient care services.
Skills and Qualifications:
- Education: Bachelor's degree in nursing, healthcare administration, or a related field; a Master's degree is preferred.
- Experience: 5+ years of experience in care management or case management, with at least 2 years in a leadership role.
- Skills: Strong leadership, communication, and organizational skills; proficiency in care management software and tools; in-depth knowledge of healthcare regulations and payer systems.
