Care Management Manager

Care Management Manager is responsible for overseeing and coordinating the care management program, responsible for coordinating patient care, ensuring that patients receive appropriate care and services efficiently and effectively. This role involves developing care management strategies, supervising care managers, and collaborating with healthcare providers to ensure quality care delivery.

Key Responsibilities:

Leadership and Supervision:

• Lead a team of care managers, and support staff, providing guidance, training, and performance evaluations.
• Oversee the daily operations of the care management department, ensuring compliance with organizational policies and regulations.
• Coordinate staffing and scheduling to ensure adequate coverage for patient care needs.

Care Coordination and Planning:

• Develop, implement, and monitor patient care plans to ensure they meet the needs of patients, improve health outcomes, and reduce hospital readmissions.
• Collaborate with multidisciplinary teams (e.g., doctors, social workers) to develop individualized care plans for patients with complex medical needs.
• Review patient data to assess risks, evaluate progress, and adjust care plans as necessary.

Program Development and Improvement:

• Create and implement strategies to improve care management processes, focusing on patient-centered care and reducing unnecessary healthcare costs.
• Monitor performance metrics (e.g., patient satisfaction, readmission rates) and adjust care management strategies based on these outcomes.
• Lead initiatives to enhance care transitions, discharge planning, and chronic disease management.

Regulatory Compliance:

• Ensure that all care management practices comply with healthcare regulations, accreditation standards, and payer requirements.
• Oversee documentation and reporting to ensure accurate and timely submission of data for audits and compliance.

Collaboration with External Partners:

• Establish and maintain partnerships with community resources, and other external service providers to enhance care coordination.
• Work closely with insurance companies to ensure patients receive necessary services while managing costs effectively.

Budget and Resource Management:

• Manage departmental budgets, ensuring efficient use of resources.
• Identify opportunities to reduce costs while maintaining high-quality patient care services.

Skills and Qualifications:

• Education: Bachelor's degree in nursing, healthcare administration, or a related field; a Master's degree is preferred.
• Experience: 5+ years of experience in care management or case management, with at least 2 years in a leadership role.
• Skills: Strong leadership, communication, and organizational skills; proficiency in care management software and tools; in-depth knowledge of healthcare regulations and payer systems.
