We are currently hiring a full-time LVN for our West Ventura County office. In this role you will be assisting the Director of Nursing with both Home Care and Skilled clients and patients.

REQUIREMENTS:

  • Graduated from a National League of Nursing accredited practical or vocational school of nursing.
  • Unencumbered license as a Vocational Nurse in California.
  • 1 year of nursing experience preferred. Community, home health or medical experience preferred.
  • 1 year of office experience.
  • Proficient with Microsoft Office and computer applications.
  • Comply with the state nurse practice act.
  • Comply with accepted professional standards of practice.
  • High level of interpersonal skills.
  • Excellent customer service skills and ability to deal tactfully with clients, other team members and community.
  • Treat clients, patients, partners, referral sources, team members and the public with courtesy, respect and presents a positive public image.
  • Empathy for the needs of the client.
  • Works as a team member.
  • Self-directing with the ability to work with little direct supervision.
  • Licensed driver with current auto insurance.
  • Recent physical, or ability to obtain one 3-weeks after hire date.
  • Negative TB test or chest x-ray.
  • BLS Certification
  • On-call, rotation


RESPONSIBILITIES:

  • Case Management (intakes, nurse scheduling, patient contact, account contact)
  • Daily confirmation calls with nurses and patients; regular patient touch points to ensure care satisfaction.
  • Discharge patient files for home care two weeks after the last visit.
  • Follow-up with MD Office: Obtain signed orders and address nurse concerns by following up with the MD office.
  • Onboard new nurses, facilitate ongoing communication, and monitor TigerConnect messages.
  • Schedule and paperwork preparations for performance reviews.
  • Monitor patient care authorization dates and manage consumption for timely renewals.
  • Create and Monitor teams on TigerConnect.
  • Compliance: Ensure Joint Commission and BrightStar Care's internal policies are followed, including managing inquiries and handling supplies.
  • Documentation: Maintain up-to-date patient records, including uploading documents into Bamboo and creating summaries for Nurse evaluations.
