Job Description

PURPOSEOFPOSITION:
The CDTM Pharmacist is responsible for ensuring medication use is aligned with evidence-based best practices and regulatory standards. To directly manage medication safety systems, regulatory compliance initiatives, and patient quality and disease outcomes programs.
Under the direction of the collaborative clinical team lead, performs clinical services related to the review, evaluation, and management of drug therapy to a patient, being treated by a physician for a specific disease or associated disease states, in accordance with a written agreement or protocol with a voluntarily participating physician.
To coordinate/participate in activities related to improving outcomes for targeted disease management initiatives leading to improved mental and physical health (e.g., long-acting injectable medications, metabolic syndrome)

POSITIONSUMMARY:

The CDTM Pharmacist is responsible for providing comprehensive clinical pharmacy services for King's County patients on behavioral health medications including pharmacokinetic and therapeutic consultation and support for primary care pharmacotherapy as needed.

AREASOFRESPONSIBLITIES:
1. Operates under defined protocols based upon agreement(s) with Collaborating Provider(s) and accordance with the policies, procedures and protocols of NYC Health+ Hospitals (H+H) and New York State (NYS) guidelines
2. Performs comprehensive medication review and reconciliation and resolves medication-related problems in order to optimize drug therapy
3. Provides patient education on self-management and assist patients and providers with medication assess issues to ensure understanding of care plan
4. Ensure medication errors and ADRs are reported and pathways developed to prevent future occurrence
5. Provides educational programs for health care staff regarding medication use; provides training as measured by the success of the staff to accomplish identified goals and improve patient care
6. Support of quality improvement projects and research initiatives
7. Performance of medication use reviews to assess cost effectiveness and medication appropriateness
8. Performs related duties, as directed

SUMMARYOFDUTIESANDRESPONSIBILITIES:
Implements and modified drug therapy of individual patients or groups of patients (patients with diabetes, asthma, hypertension, etc.)
Adjusts the management of drug strengths, frequency or route of administration and/or substitutes authorized drug in accordance with the Drug Therapy Protocol with the Collaborating Provider
Evaluates clinical laboratory test related to the drug therapy management for the specific disease or disease state specified within the Protocol and orders such clinical laboratory tests, only if specifically authorized by the protocol and to the extent necessary to discharge their responsibilities under the law.
Orders and/or performs routine patient monitoring functions, as necessary, in drug therapy management including collecting and reviewing patient histories, conducting relevant physical assessments and ordering or

checking of patient vital signs, including pulse, temperature, blood pressure and respiration, to the extent necessary to discharge responsibilities set forth in the protocol.
Documents modifications made to the patient drug therapy in each participating patient's record and notifies other treating providers, as required
Performance of medication use reviews to assess cost effectiveness and medication appropriateness.
Support of quality improvement projects and initiates/coordinates MUEs for P&T presentation; discusses findings.
In the absence of the supervisor, may assist in the supervision of a Pharmacy unit or function.
Maintains responsibility for both the clinical and the distributive activities of the Pharmacy Department as necessary.
Assists with staffing as deemed necessary.
Perform other related duties as required.

Minimum Qualifications
1. A valid New York State license and current registration to practice pharmacy; and
2. Must be certified to practice Collaborative Drug Therapy Management (CDTM) by the New York State
Education Department (NYSED).
