Job Description

Purpose of Position:

The Virology Clinic CDTM Pharmacist will provide outpatient services for participants in the HIV/ Aging Program and to all patients serviced by the clinic, such as patient education, prevention of drug interactions and side effects, and assessment of possible polypharmacy

Roles and Responsibilities:
  • In collaboration with the Virology Grants Director, Assistant Director and Medical Director:
  • Provide outpatient Ambulatory Care Services to treat the complex needs of Aging PWH
  • Work with clients that experience polypharmacy or are at risk of polypharmacy
  • Provide patient education and strategize with patients on maintaining medication adherence
  • Work with providers and clients to identify potential drug-drug interactions and medications that have a less favorable side effect profile in a geriatric population
  • Following agency Collaborative Practice Agreements for Pharm D and Collaborating Physicians, support the adjustment and management of medications, evaluate clinical lab tests, and monitor patient's drug therapy management


Minimum Qualifications
1. A valid New York State license and current registration to practice pharmacy; and
2. Must be certified to practice Collaborative Drug Therapy Management (CDTM) by the New York State
Education Department (NYSED).

Department Preferences

Experience in working with people living with HIV (PLWH), adults over 50 years of age or other relevant populations
