Certified Occupational Therapy Assistant (COTA)
Connected Home Health is seeking an experienced Certified Occupational Therapy Assistant to serve patients in Portland, Oregon, and surrounding communities. We are looking for a seasoned Certified Occupational Therapy Assistant who is willing to help develop best practice guidelines and will share our passion, energy, and dedication to our core values.
About the Opportunity
Participates in the ongoing evaluation of patient's functional status as delegated by the registered Occupational Therapist.
Participates in the ongoing evaluation of the home environment for hazards or barriers to more independent living
Participates in teaching new skills or retraining patients in once familiar daily activities that have been lost due to illness or injury, in accordance with Connected Home Health policy
Maintains appropriate clinical records, clinical notes, and reports the registered Occupations Therapist an y changes in patient's condition
Submits clinical documentation in accordance with Connected Home Health policy
Follows treatment program and goals for improved patient function as established by the registered occupational therapist. Documents such communication in accordance with Connected Home Health policy
Attends and contributes to in-services, case conferences, and other meetings as require
Identifies patient and family/caregiver needs for other home care services. Consults with supervising registered Occupational Therapist and assists with necessary referrals, as appropriate
Participates in instructing patient's family/caregiver and other Connected Home Health care personnel in patient's treatment regime as delegated by the registered Occupational Therapist
Requirements
Graduate of an occupational therapy assistant curriculum accredited jointly by the American Occupational Therapy Association
Certified with the National Certification Examination of the American Occupational Therapy Association
Licensed by the State Board of Healing Arts in accordance with State licensure laws
A minimum of two years of clinical experience as a Certified Occupational Therapy Assistant
Current BLS for Healthcare Professionals
Must be a licensed driver with an automobile that is insured in accordance with state or Connected Home Health requirements and is in good working order
