Position Summary:

The Change Management Specialist will work closely with clinical and administrative teams to contribute to leading change management initiatives to facilitate the adoption of new processes, technologies, and care delivery models across the organization. Leveraging their expertise in change management methodologies, healthcare operations, and project management, they will champion a culture of innovation, resilience, and patient-centered care to support HealthPoint's mission of advancing health equity and access for all.

Compensation is dependent on skills and experience.

Your contribution to the team includes:
  • Develop and implement change management strategies and plans to support the successful execution of organizational initiatives, such as the implementation of new electronic health record systems, quality improvement projects, and care coordination programs.
  • Collaborate with the Enterprise Project Management Office (EPMO) and the Transformation Process and Improvement Office (TPIO) to align change management efforts with project management methodologies, quality improvement initiatives, and Lean process improvement principles.
  • Conduct comprehensive assessments of organizational readiness for change, identifying potential barriers and resistance factors within clinical workflows, administrative processes, and staff culture, and develop targeted strategies to address them proactively.
  • Lead stakeholder analysis and engagement efforts to ensure alignment, build support, and foster collaboration among clinical providers, staff members, community partners, and leadership teams.
  • Partner with leadership to interface with the Senior Leadership Team (SLT) to provide regular updates on change management initiatives, solicit feedback, and secure leadership buy-in and sponsorship for key organizational changes.
  • Design and deliver effective communication plans to keep stakeholders informed, engaged, and motivated throughout the change process, utilizing various communication channels and culturally competent messaging.
  • Partner to develop and execute comprehensive training programs to equip clinical and non-clinical staff members with the knowledge, skills, and resources necessary to adapt to change, optimize workflows, and deliver high-quality, patient-centered care.
  • Monitor and evaluate the effectiveness of change management initiatives, leveraging feedback mechanisms, performance metrics, and patient outcomes data to measure adoption, and drive continuous enhancement.
  • Provide coaching and support to clinical leaders, department managers, and frontline staff to enhance their change leadership capabilities and empower them to effectively navigate and lead their teams through transitions.
  • Stay abreast of industry trends, emerging technologies, and best practices in healthcare delivery and change management, incorporating new insights and innovations into organizational change initiatives to drive sustainable improvements in patient care and operational efficiency.
  • Maintain good attendance, is punctual and works full scheduled shift is a condition of employment.

  • Other duties as assigned by supervisor.
  • Attend staff meetings, in-service meetings and participate in agency committees and task force activities as required.

Must have's you'll need to be successful:
  • Change Management certification required.
  • Knowledge of computer equipment, intermediate level of Word, Excel, and Outlook, and knowledge in Project and Change Management software required.
  • High School Diploma/GED and Bachelor's Degree (BA/BS), and three (3) to five (5) years' experience leading change management in another healthcare organization or equivalent combination of education and experience. Master's Degree in Business Administration, Organizational Development, Health Administration or other related field preferred.
  • Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to write reports, complex papers, speeches and articles using original or innovative techniques or style. Ability to make effective and persuasive speeches and presentations on controversial or complex topics to top management, clients, external groups, and/or boards of directors.
  • Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems. Work situations are vaguely defined, often unprecedented, and unique in character. Problems are very complex, abstract, conceptual, and long-term in nature. There is a continual requirement for strategic, innovative thought and synthesis of information as it pertains to work situations.

Proof of vaccination for COVID-19 is required, prior to start. HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination. If you have received the Johnson & Johnson vaccine, we ask that you provide documentation demonstrating proof of an alternate COVID vaccine or vaccine series. All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment.
