Job Title: Clinical Care Leader, Long Term Care
Job Specialty: Nursing
Job Duration: 13 weeks
Shift: 8:00 am - 4:30 pm
Guaranteed Hours: 40 hours per week
Experience: Minimum of two years of long-term care or geriatric nursing experience preferred
License: Must hold an unencumbered RN license with the State Board of Nursing where the practice is occurring and/or possess multi-state licensure if in a Nurse Licensure Compact state
Certifications: Basic Life Support (BLS) required
Must-Have: RN license, BLS certification, Long-Term Care experience preferred
Job Description:
- Utilize the nursing process, including assessment, diagnosis, outcomes/planning, implementation, and evaluation, to provide individualized nursing care in a home setting.
- Collaborate with inter-professional colleagues to plan, implement, and evaluate care.
- Document resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations.
- Use technology to enhance the quality of resident care and collaborate effectively with inter-professional team members.
- Delegate and supervise unlicensed personnel, assuming overall nursing accountability for a resident assignment.
- Serve in leadership roles on department/organizational task forces and committees.
- Demonstrate competency and practice within the full scope of nursing expertise/knowledge, utilizing appropriate age and population-specific standards in the care area or unit.
- Possess effective communication skills to collaborate, negotiate, and resolve conflicts with peers, inter-professional colleagues, residents, and families.
- Exhibit strong clinical reasoning and critical thinking skills to analyze data and respond appropriately to changing situations.
- Coordinate care, conduct resident assessments and education, triage, and various other nursing interventions.
- Function within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure.
- Adhere to the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association.
- Educate and train others on operations, ethics, and regulations within the home healthcare industry.
- Establish and maintain productive working relationships within and outside the area.
- Promote an organizational culture that balances individual initiative with team collaboration and sharing of successes.
- Reinforce positive contributions and recommend adjustments for improving outcomes.
- Maintain a safe environment for patients, families, staff, residents, and visitors while working collaboratively with the multi-disciplinary healthcare team to provide the best outcomes.
- Train others on main considerations and issues related to laws and regulations in healthcare practice implementation.
- Use active listening skills and probing techniques to understand the cause of a conflict.
- Conduct patient care activities including but not limited to handling contaminated sharps and specimens, unpacking, storing, or disposing of hazardous drugs, and potentially being exposed to biohazards.
