Job Summary

The Clinical Coordinator works in collaboration with leadership to support the Patient Access and Flow program, specifically enabling efficient patient flow across the organization, and supporting the Virtual Nursing Team (VNT).

As a corporate leader, the Clinical Coordinator collaborates closely with Access & Flow leadership and inter-professional teams (i.e., VNT, Bed Allocation, Discharge Nurses, Scheduling Office) to support staffing needs, maintain high standards of patient care, and ensure optimal patient outcomes. Key responsibilities include supporting and/or leading daily patient placement meetings, monitoring key performance indicators, and addressing any barriers to patient access or flow.

The Clinical Coordinator is responsible for implementing evidence-based practices, supporting staff development, and playing an active role in quality improvement initiatives. They will also collaborate on recruitment, performance evaluations, and continuous improvement activities that enhance the patient and staff experience.

Primary Responsibilities

* Patient Access and Flow: Maintains presence on the units to coordinate patient flow activities across the organization to reduce delays, ensure timely admissions, and optimize bed utilization.
* Collaboration: Liaise with leaders, physicians, nurses, support services, and allied health professionals to promote a smooth and efficient patient flow and care process.
* Staffing Oversight: Collaborate with the Manager of Access & Flow regarding daily staffing needs and assist with scheduling and resource allocation.
* Performance Management: In collaboration with leadership, participate in the recruitment, onboarding, and performance evaluations of team members, inclduing probationary reviews/interviews. Assist in implementing strategies for continuous learning and development.
* Clinical Leadership: Lead the implementation of evidence-based practices and support the professional development of clinical staff. Provide support for performance discussions and conflict resolution in order to optimize team function and integration.
* Quality Assurance: Monitor standards of care, participate in quality assurance initiatives, and collaborate with inter-professional teams to improve patient care outcomes. Actively participate in the Accreditation process and assist in the development of program goals and objectives.
* Communication and Coordination: Serve as a key point of communication for the Patient Access & Flow Program, inlcuding the VNTs, ensuring that patient flow-related issues are addressed and resolved efficiently.
* Change Facilitation: Implement strategies that facilitate change, encourage staff engagement, and improve patient flow processes in alignment with organizational goals. Liaise with leaders, physicians, staff, students and volunteers.

Qualifications

* RN Certificate of Registration from the College of Nurses of Ontario
* Bachelor's Degree in Nursing Preferred
* Minimum of 3 to 5 years of acute care clinical experience with knowledge of clinical practice
* Minimum 2 years progressive administrative experience (team leader/Charge Nurse/ supervisor, etc) in a clinical setting preferred
* CNO, RNAO, other professional affiliations as appropriate
* Demonstrates a strong patient and customer focus philosophy in all interactions
