Role Scope:

Our Enrollment team focuses on the end-to-end process of identifying, attracting, engaging with, and enrolling new participants into Habitat's PACE program.

We are looking for a Director, Pre-Enrollment Operations to define and execute the clinical blueprint for the Enrollment function. This role will report directly to Habitat's Vice President of Growth.

Responsibilities will include:
  • Define workflow, process maps, and tools to support the clinical assessment process for prospective PACE participants, including operationalizing level of care (LOC) criteria and Health & Safety (H&S) criteria.
  • Coordinate with Habitat Health's Marketing teams to ensure that marketing messaging is clinically informed and accurate.
  • Define and operationalize Standard Operating Procedures (SOPs), Committees, and related workflows necessary to oversee prospective participant IDT / H&S denials.
  • Define and operationalize medical record review workflow to inform the process of establishing whether a prospective participant meets eligibility for PACE.
  • Establish key clinical pre-enrollment metrics.
  • Support development, refinement, and implementation of pre-enrollment strategy and related clinical initiatives, including Central/Center pre-enrollment staffing models.
  • Provide job aids, training materials, checklists and other related tools, as well as coaching as necessary, to Habitat Health Nursing staff involved in the pre-enrollment process.
  • Respond to any internal or external audits of the Habitat Health eligibility process.

Qualifications:
  • A bachelor's degree in nursing (BSN) is preferred; professional experience may be substituted.
  • 7+ years of relevant experience, such as in health care delivery at a high-growth startup, patient growth, and community outreach. Prior experience at a PACE organization is strongly preferred.
  • Demonstrated ability to be a self-starter, with the ability to excel in a fast-paced, unstructured work environment, comfort operating in ambiguity, and a bias to action.
  • Excellent analytical and project management skills.
  • Strong interpersonal and written/oral communication skills; high EQ.
  • A strong passion for our mission, with a high degree of empathy for the populations we serve.

Compensation:

We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $150,000 - $175,000 and is bonus-eligible. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.
