Job Description
Minimum Qualifications
1. Valid New York State license and current registration to practice as a Registered Professional Nurse(RN) issued by the New York State Education Department (NYSED); and Bachelor of Science inNursing degree from an accredited college or university; and four (4) years of acute care experience; or
2.Valid New York State license and current registration to practice as a Nurse Practitioner (NP) issued by the NYSED; and two (2) years of experience, as described in "1" above; or
3.Valid New York State license and current registration to practice as a Physician Assistant (PA)issued by the NYSED; and two (2) years of experience, as described in "1" above; or
4.Foreign Medical Graduate; and, two (2) years of medical records review or utilization and case management experience; or
5.Successful completion of education that leads to a medical degree; and two (2) years of experience as described in "4" above.

Job Description
- Conduct initial reviews & compute working DRGs while Identifying CCs and MCCs.
- Apply knowledge of payer requirement for appropriate DRG assignments such as MS DRG.,APR, etc.
- Develop and send written and / or verbal queries to providers for their response.
- Identify opportunities for documentation improvement to ensure appropriate ICD-10 code assignment.
- Communicate with the coders/ Validator to resolve discrepancies between working and final DRG.
- Collaborate with physician champions to promote clinical documentation improvement among peers.
- Track and Monitor Initial reviews, queries and provider responses and ensure timely and optimal result
- Conduct continuous reviews on cases.
- Formulate compliant queries to seek clarification on documentation.
- Generate reports to monitor CDI process as well as productivity reports and provider response
- Review the 3M 360 Collaborate M-Modal system to identify chronic diagnoses that are
- Hierarchical Chronic Conditions (HCCs) and Chronic Risk Groups. (CRGs)
- Send queries/notifications to the providers to add chronic Diagnoses to the Provider notes.
