**The Details**
We are seeking a driven and knowledgeable Clinical Manager to join our incredible team in San Francisco, California! In this role, you will play a vital part in supporting the Director of Patient Care Services (DPCS) and in driving the success of our clinical team. You will be responsible for guiding the clinical team on proper processes as they care for our patients. We're looking for someone with exceptional leadership experience, a solid understanding of home health care, and qualities such as compassion, patience, and service orientation.
This is a full-time position from Monday to Friday.
**Responsibilities:**
- Supervise hospice nurses, LVNs, and other patient care team members.
- Ensure high-quality patient care is delivered and that care plan goals are met.
- Review documentation, patient satisfaction reports, and care plans.
- Participate in and oversee team conferences, conducting or participating in multidisciplinary team meetings and reviewing patient satisfaction reports.
- Resolve patient and family member concerns.
- Perform disciplinary actions and employee training; conduct performance assessments for employees.
- Ensure compliance with company policies and procedures.
- Complete other administrative tasks as assigned.
- Conduct field visits to patients as needed.
- Supervise field visits with hospice patient care providers.
**Qualifications:**
- Current CA RN license.
- Previous leadership or supervisory experience (preferred).
- Familiarity with clinical complexities.
- Strong multitasking and time management skills.
- Excellent verbal and written communication skills.
- Exceptional interpersonal skills.
- Team-oriented mindset.
