About the Role:

The Clinical Manager at United Hospice will play a pivotal role in overseeing the delivery of high-quality patient care within our hospice services. Reporting to the Director of Nursing, this position is responsible for ensuring that all clinical operations align with regulatory standards and best practices, ultimately enhancing the services to our patients and their families. The Clinical Manager will lead a team of healthcare professionals, providing guidance, support, and training to ensure optimal delivery of services. Additionally, this role involves collaborating with interdisciplinary teams to develop and implement care plans tailored to individual patient needs. The successful candidate will be instrumental in fostering a compassionate and supportive environment for both staff and patients, ensuring that our mission to honor life, give care and bring comfort at end-of-life care is upheld.

Salary for this position is between $100,000-115,000 per year, and is commensurate with applicable experience.

Minimum Qualifications:
  • Bachelor's degree in Nursing is a must; Master's degree in Nursing, MPH or equivalent is preferred
  • Current NYS nursing license is required; within one year of hire, will sit for and pass the Hospice and Palliative Care Nursing Certification Exam
  • Minimum of three years supervisory experience required with at least five years clinical nursing experience in either hospice or hospital-based med/surg nursing, with electronic medical record competence

Responsibilities of the Clinical Manager will include, but not be limited to:
  • Provide administrative supervision for all Community HHAs and RNs, ensuring performance standards are maintained according to all internal and external regulatory mandates
  • Participate in the Administrative On-Call (AOC) weekend rotation
  • Provide coverage for the Director of Nursing (DON) in their absence
  • Schedules Community RNs and HHAs to include weekend and holiday coverage
  • Approve PTO requests for direct reports
  • Ensures mandatory HHA supervisions are conducted in a timely and compliant basis
  • Participates in New Hire Orientation
  • Conducts patient record reviews to include EMR; document and track finding and devise plans of improvement and corrective action as needed; communicates findings to DON
  • Prepare scheduled and special performance evaluations for all direct reports; utilizes UH's internal progressive discipline protocols in place when needed
  • Cooperate with and participate in all investigations as needed
  • Participate in and remains compliant with required QAPI program and hospice sponsored in-service training
  • Incorporates the organization's commitment to customer service and continuous quality improvement into daily work habits, including promoting positive co-worker and teamwork relationships; embrace our shared core values and code of conduct as set forth by the Board of Directors
  • Provide leadership to all direct reports by serving as mentor, creating a positive work culture and cohesive team
  • Promote the professional growth of all Community Nursing staff and Home Health Aides through coaching, mentoring and continuous training
  • Create a positive environment conducive to leadership and development and progressive nursing practice by demonstrating decision making and problem solving skills
  • Assist the nursing staff in establishing immediate and long-term goals, setting priorities and in developing a patient/family plan of care
  • Communicates all pertinent issues to the Director of Nursing
  • Supervise nursing staff and HHAs in the community to ensure that performance standards are upheld and that all individuals are working in accordance with organizational requirements, missions, goals, and principles
  • Cover RNs on an as needed basis, coordinating all aspects of patient's care in accordance with professional nursing standards and practice
  • Review and evaluate each direct report's patient cases through a variety of means such as supervisory field/home visits, conferences and record review. Discuss supervisory impressions, instruct and guide Community HHAs and RNs to promote effective performance and delivery of quality hospice care services
  • As an integral member of the Inter-Disciplinary Team (IDT), leads case conference meetings to facilitate coordination of services and clinical care interventions
  • Creates opportunities for direct reports, through field visits and individual supervision to develop skillsets and foster independence
  • Travel as needed to conduct field supervision of community nursing staff and HHAs.
  • Accepts other responsibilities as assigned

Skills:

The required skills for this role include strong leadership abilities, which are essential for guiding and motivating the clinical team to deliver exceptional patient care. Effective communication skills are crucial for facilitating discussions among patients, families, and healthcare providers, ensuring that everyone is aligned on care plans. Critical thinking and problem-solving skills will be utilized daily to assess patient needs and make informed decisions regarding care adjustments. Will require proficiency in MS Office Suite, EMRs and learning new software systems. Additionally, organizational skills are necessary for managing multiple patient cases and ensuring compliance with hospice regulations. Preferred skills, such as knowledge of hospice quality standards, will enhance the ability to monitor and improve patient outcomes effectively.
