This position is critical for the overall operation and success of the clinic.
Essential Job Duties:
- Operate a high-performing, well-equipped, and safe clinic, ensuring workflow efficiency, clarity, and adherence to protocols.
- Hire, retain, coach, develop, and supervise RN Case Managers, Clinic RNs, Medical Assistants, and other clinic team members, ensuring team engagement and well-being.
- Analyze data to identify trends, strengths, weaknesses, and opportunities. Perform root cause analysis, evaluate care delivery processes, and recommend changes to strategies, workflows, and policies for continuous improvement.
- Ensure nursing and clinic staff deliver and document interventions according to participants' care plans, including responding to physician orders, completing assessments, and participating in care planning.
- Evaluate the quality and effectiveness of coordination and services in collaboration with medical practice leadership, providers, case managers, and other relevant interdisciplinary team members.
- Serve as the audit lead for clinical and nursing elements during regulatory audit reviews in collaboration with relevant departments.
- Develop and monitor corrective action plans to ensure ongoing compliance.
Job Requirements:
- Bachelor's degree in nursing or other healthcare-related field preferred; professional experience may be substituted.
- Unencumbered registered nurse (RN) state license required.
- Minimum of two (2) years of experience overseeing an ambulatory clinic operation, including RN, LVN, and MA staff; equivalent nurse leader experience in a high-volume, outpatient setting may be substituted.
- Minimum of two (2) years of experience in a clinical setting with a frail or elderly population.
