Compensation (Pay): $55.69 to $80.75/hourly, based on experience. There will be a $2.00 per hour lift added to this position.

Provides comprehensive medication management and related care for patients. May also work in prescription dispensing in Peak Vista Community Health Centers pharmacies.

Education/Experience:
Education: Doctor of Pharmacy degree or completion of at least 5 years of experience as a licensed pharmacist.
Work experience: No experience required with Doctor of Pharmacy Degree; minimum of 5 years' experience as a licensed pharmacist required if degree is not Doctor of Pharmacy; experience in ambulatory care clinic preferred.
Additional qualifications: Completion of certification program or post-graduate education specific to an area of clinical practice required. ACPE accreditation of certification program preferred.
Computer Skills: Intermediate - ability to access the intra/internet to manage timecard, review policies and procedures, and read company communications; use e-mail to communicate with co-workers, leadership, and other departments; use and create a variety of templates, complex tables, merges; manage table data, sort and filter merges, and also perform basic work with existing Macros; customize toolbars, import and insert graphs, embed Excel data, and elaborate reports; work with multiple worksheets, filter data, use integrate functions, and manipulate databases; customize templates and the PowerPoint environment, and to make a presentation interactive by using hyperlinks and action buttons. Use electronic health record to review patients, document encounters, create task, order and document labs, order prescriptions
Certificates and Licenses: Required: Registered Pharmacist with a current license in the state of Colorado. Meets all Colorado State Board of Pharmacy requirements to engage in Collaborative Drug Therapy Management.

Essential Duties and Responsibilities
- Performs all essential duties and responsibilities of Pharmacist.
- Exhibits the traits of clinical professionalism: responsibility, accountability, commitment to excellence, respect for others, honesty and integrity, and care and compassion. Acts in the best interest of individual patients and patient populations within the context of legal and ethical parameters.
- Demonstrates knowledge of Colorado State Board of Pharmacy collaborative pharmacy practice regulations and is eligible for participation under these regulations.
- Follows Peak Vista collaborative practice agreement protocols, policies, and procedures for ambulatory care centers in which they practice.
- Processes new patient referrals as delegated for ambulatory care centers in which they practice.
- Maintains competence in clinical problem-solving, judgment, and decision-making; communication and education; medical information evaluation and management; and management of patient populations
- Exhibits excellent communication skills and ability to effectively educate patients on disease states and medication use.
- Works in collaboration with medical providers to deliver comprehensive medication management that optimizes patient outcomes.
- Conducts medical record reviews by interpreting and analyzing subjective and objective information documented in electronic health record and by meeting with patient/caregivers to obtain additional prescription or past medical history including medication adherence, allergies, intolerances, attitudes towards medication therapy, and patient-specific goals.
- Performs capillary blood draws for in-office point-of-care assessment, interprets results, and alters patient treatment plan based on the results as applicable to the clinic in which they work
- Assesses the appropriateness of current medications regarding current or past health conditions, indications, evidence-based guidelines, and the therapeutic goals of each medication.
- Evaluates the effectiveness, safety, and affordability of medication plans.
- Develops and implements a patient-centered plan to optimize medication therapy and educates the patient/caregivers (both verbally and in writing) to ensure understanding of the care plan, optimize adherence, minimize financial burden, and improve therapeutic outcomes.
- Monitors and assesses disease state changes in response to medication therapy and makes changes to optimize therapy and ensure individual goals are achieved.
- Demonstrates effective, accurate, and timely documentation to communicate context of appointment encounters across health system.
- Documents assessments and plans into electronic health record in compliance with the accepted standards for documentation (and billing, where applicable) within the health system.
- Maintains proficiency in medication ordering system within electronic health record.
- Participates in continuing professional development activities that enhance direct patient care practice abilities specific to the clinical area in which they practice.
- Attends and participates in clinic meetings.
