Job Title: Center of Excellence Pharmacist

Department: Operations

FLSA: Exempt

General Function:

Following all state and federal statutes and regulations verifies prescriptions and counsels patients. Develops, reviews, and evaluates clinical guidelines and Drug Utilization Reviews and helps develop training programs. The Center of Excellence Pharmacist assumes responsibility for managing operations and personnel issues without the Pharmacist in Charge (PIC).

Primary Duties and Responsibilities:
  • Calls provider's offices for questions, clarifications, potential drug interactions, adverse reactions, and other clinical interventions. Establishes and maintains effective professional relationships with providers.
  • Monitors patients' drug therapies and works with healthcare providers to ensure the best possible care is being given.
  • Assists with supervising, directing, developing, and training pharmacy technicians, student interns, and patient care specialists.
  • Enforces all state, federal, and local laws about drug dispensing, patient information, and company policies and procedures. Reports violations to supervisor and/or Pharmacy Manager.
  • Takes verbal prescription orders over the phone.
  • Counsel patients, caregivers, providers, and others on medications as needed.
  • Calls insurance companies for billing authorizations as needed.
  • Participates in rotating schedule and on-call duty as assigned.
  • Represents the Company through membership and active participation in pharmaceutical professional groups, associations, and organizations.
  • Maintains an awareness of developments in the pharmacy industry related to job responsibilities and integrates them into practice.
  • This is an office-based position.


Clinical Pharmacist Primary Responsibilities:
  • Maintains privacy at all times for patient information.
  • Operates pharmacy systems and software to obtain patient and drug-related information for initial assessment, patient care planning, appropriate documentation, and follow-up following ACHC/URAC accreditation standards.
  • Offers recommendations for improvement and assists with implementation to achieve maximum efficiency.
  • Reviews pharmacy department policies, procedures, and inventory to ensure compliance with federal and state pharmacy regulations.
  • Ensures compliance with DEA and state Board of Pharmacy regulations regarding the maintenance of records and documents.
  • Ensures safe and effective use of medications by providing medication order verification, dosage regimen design, effective administration, and drug therapy monitoring services.
  • Provides information and guidance about medications' effects, dosage amounts, and potential side effects. Help plan effective drug therapy programs using patient history to make clinical decisions.
  • Analyzes and investigates concerns from patients or providers regarding therapies or services. Assists with the resolution of customer service opportunities regarding assigned departments.
  • Assists with the implementation, follow through, and administrative requirements associated with new clinical programs and all medications, which require specialized monitoring via REMS, MTM, patient plan of care, or manufacturer-based requirements.
  • Will be considered a subject matter expert (SME) to assigned designated areas and/or conditions.
  • May assist with Fulfillment Pharmacy duties as needed.
  • Participates in on-call and shift rotation schedule.
  • Actively participates in the organization's Quality Management Program and organizational-wide Quality Improvement Projects to improve quality of care/services and patient/customer satisfaction.
  • Adheres to all company policies as indicated in the handbook and directives issued by management. Has reviewed the Policy and Procedure manual.
  • May perform supervisory or work delegation functions as needs dictate.


Center of Excellence Pharmacist Responsibilities:
  • Responsible for the authorization of prescription order entry. Conducts Quality Assurance and verifies the accuracy of all prescriptions.
  • Answers Patient Care Pharmacist phone line and direct calls.
  • Performs initial clinical assessments according to drug and/or disease state.
  • Assists with the review of Prior Authorization and Appeals requests.
  • Improves pharmacy prescription sales, profit, patient count, and professional image by ensuring the accurate filling of prescriptions and courteous and efficient service to all customers.

Educational and Experience Requirements:
  • A Bachelor of Science in Pharmacy, Doctorate preferred
  • Six months or less of similar work experience
  • Must obtain pharmacist licensure within the assigned state
  • Must be willing to obtain reciprocity of licensure and travel to locations at the discretion of the Pharmacy Manager
  • Working knowledge of the healthcare industry
  • HIPAA trained and /or the ability to work with and protect highly confidential patient and employee information


Licensure and Certification Requirements:
  • Registered Pharmacist (RPh)
  • The current license must be free from restriction and in good standing within the state of practice
  • Additional state Pharmacist licenses are highly desirable, but willingness to obtain additional licensure will be required
  • Certification or expertise in a specific clinical area preferred


Physical Requirements:
  • Constantly operates in the fulfillment pharmacy, standing up to 7 hours daily.
  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
  • The ability to observe details at close range (within a few feet of the observer).

Normal office environment

May assist with Fulfillment of Pharmacy duties.

Equipment Used to Perform the Job:

Computers, keyboards, mouse, monitors, fax, and/or headsets for phone work. Software specific to the position, including but not limited to Microsoft Outlook and Skype. Must lift and traverse the area to move paper and supplies to use the equipment.

Contacts:

Frequently interact with co-workers, intradepartmental staff of the Company, managers, leaders, Vendors, and customers. High interactions with physicians and the medical community to support the sales and perfection of the referral to fill.

Confidentiality:

The position Interactions with medical personnel, and patients and accesses proprietary and medical (HIPAA) information in various interactions and matters.

Candidates must be able to pass a pre-employment drug test, background check, and health screening (if applicable).
