What We Are Looking For
The Clinical Quality Pharmacist will bring specialized skill and perspective to the management of the SAC Health population with specific attention to helping with chronic disease management. Particular areas of interest will be in Asthma treatment, Diabetes treatment, and Hypertension treatment. The pharmacist will work with our multidisciplinary Population Health team to help attain data driven goals in our SAC P4P and UDS quality metric programs. This will be accomplished through both direct patient contact and care as well as through program development and implementation.
Schedule: 5 days per week, 8 hours per day, Monday - Friday, 8:00 a.m. - 4:30 p.m. | Location: Brier Clinic, San Bernardino, CA

ESSENTIAL FUNCTIONS AND DELIVERABLES
- Conduct comprehensive appraisals of patient's health status by taking health histories, drug histories and performing physical examinations necessary to assess drug therapy.
- Evaluate drug therapy through clinical assessment, direct patient care involvement, subjective and objective findings relating to patient's responses to drug therapy, and communicating and documenting those findings and recommendations to appropriate individuals and in appropriate records (i.e., patient's medical record).
- Assure optimal, effective pharmaceutical care and collaborative practice for SAC Health patient population.
- Monitor and optimize drug therapy regimens in the clinic. Develop, document and execute therapeutic plans utilizing the most effective, least toxic, and most economical medication treatments as per established protocol.
- Provide ongoing primary care for chronic stable or minor acute health problems as delineated in protocols/procedures.
- Provide patient and health care professional education and medication information.
- Evaluate and document patients' and caregivers' ability to understand medication instructions and provide oral and written counseling on their medications.
- Order appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy based upon established protocols.
- Recommend medications, including initiation, continuation, discontinuation, and altering therapy, based upon established protocols.
- Conduct and coordinate research drug investigations and research under FDA guidelines and regulations along with approval by appropriate local officials.
- Analyze laboratory and diagnostic test data so as to modify drug therapy and dosing as necessary. Perform physical measurement necessary to assure the patient responds to
drug therapy. - Implement protocols approved by the physician staff regarding drug therapy. Assist in the management of medical emergencies, adverse drug reactions, and acute and
chronic disease states. - Administer medication according to pre-established protocol when requested by physicians. Identify and take specific corrective action for drug-induced problems.
- Serve as clinical advisor of drug and drug-related programs in the clinic in conjunction with the attending physician.
- Collaborate with the Quality Leadership and the Population Health Care team to achieve specific goals in the P4P program.

QUALIFICATIONS:
- Education: Doctor of Pharmacy Degree (Pharm.D.) is required.
- Licensure/Certification: Active California Pharmacist License required; current CPR/BLS certification (must be American Heart Association or Red Cross accredited program); Immunization Certification through an accredited organization (e.g. American Pharmacists Association (APhA). As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
- Experience: Minimum of two (2) to five (5) years of Hospital pharmacy practice required. 5+ years in chronic disease management required. 5+ years in quality improvement work preferred. Clinical pharmacy practice experience is preferred.
- Essential Technical/Motor Skills: Confident leader with effective management skills with ability to promote cooperation and commitment from staff to obtain goals and deliverables; Excellent planning and project management skills and ability to successfully management multiple projects; Honest and trustworthy, exhibiting sound business ethics. Excellent interpersonal skills, including leadership,
influencing, and relationship-building across functions. Bilingual English/Spanish preferred. - Interpersonal Skills: Must have excellent communications skills both orally and in writing. Must possess the ability to communicate with and relate to a diverse group of people including patients, community, and other staff. Must have strong conflict and problem resolutions skills.
- Essential Mental Abilities: Excellent problem solving and critical thinking capabilities; Diligent and thorough with an attention to detail; Strong analytical statistical, quantitative, and deduction skills and ability to make qualitative judgments of training, education and development resources in all types and formats.
- Work Eligibility: Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
