Job Summary
**FIRST REVIEW OF APPLICATIONS MARCH 4TH

This position provides community-based care coordination services to individuals who experience mental health, chemical health, and/or basic needs concerns who have been referred for services by law enforcement. This position is embedded within a local Rice County Law Enforcement Department and is responsible for providing screenings, short-term treatment planning, monitoring of client progress, care coordination, and transportation. Effective communication with local law enforcement partners and other various community agencies is essential. This position offices out of the Faribault Police Department, Northfield Police Department, or Rice County Sheriff's Office.
Examples of Duties

• Co-respond with law enforcement partners to mental health and chemical health crisis calls for service. Utilize de-escalation skills and provide expertise in immediate services available.
• Conducts screenings.
• Follow-Up care for referrals received from law enforcement partners.
• Collaborate with clients to develop short-term goals/treatment plans and monitors progress.
• Coordinates services for individuals in the community and provides assistance for inter-agency staff where necessary.
• Frequently engages with clients in the community-based settings, in private home settings, treatment/hospital care settings, and court settings.
• Provides transportation for clients.
• Communicates effectively with internal and external partners.
• Implements and improves workflows for the Rice County Community Based Coordinator Programs.
• Completes documentation, data gathering and reporting.
• Collaborates with afterhours screening services.
• Assists with training: internal and external professionals, as well as community members.
• Attends professional development training and workshops
• Performs other duties as assigned.

Qualifications

Minimum Qualifications:
The job requires a Bachelor's degree in Social Work or related human services field. Valid driver's license required. Two years' experience working with individuals with mental health and substance use disorders. Two years' experience in a correctional institution, probation department, human/social services agency or treatment/therapy setting. Must pass a finger-print based criminal history check and maintain BCA training certification throughout employment.
Preferred Qualifications:
Master's degree in Social Work or other Human Services related field and three or more years' experience working with individuals with mental health or substance use disorders. Bi-lingual in Somali or Spanish languages.
Supplemental Information

HIRING RANGE / UNION: $34.79 - $36.37 per hour / SS Unit - Grade 12HOURS: Normal working hours are 8:00am - 4:30 pm, Monday - Friday. Full-time hours equal 40 hours per week.
