Job Description

PURPOSE OF POSITION
Support Gotham Health facilities to maintain NYS PCMH accreditation for 12 Health Centers and conduct regulatory readiness assessments to all 30+ Health Centers across Gotham Health

AREAS OF RESPONSIBILITIES
Assist during regulatory readiness mock surveys.
Support the implementation of the NCQA and NYS PCMH requirements.
Support the review and update of all program related policies and procedures.

SUMMARY OF DUTIES AND RESPONSIBILITIES
1. Serve as the PCMH Lead for one of the Gotham Health Region, a liaison between the clinical and administrative leadership for the facility and the Central Office PCMH team by: a. Maintaining open lines of communication. b. Participating in meetings and other relevant discussions on initiatives/activities aligned with NYS Health Regulations. c. Coordinate trainings and or in-service across multiple subjects and areas of expertise, to enforce staff awareness of current NYSDOH and NCQA PCMH standards and requirements
2. Responsible for ensuring that Gotham primary care practices are following NCQA PCMH standards and working towards compliance for the new NYS PCMH standards by: a. Conducting site-level PCMH monthly meetings with the primary care teams, process owners and appropriate leadership. b. Conducting regular audits of data/reports, documentation, and processes ensuring that items meet the standards of government and accreditation agencies c. Conduct readiness assessment to non-accredited facilities to facilitate their initial application process, and ensure a successful accreditation outcome. e. Facilitate sharing of best practices across sites.
3. Make connections with staff and fellow colleagues by: a. Connecting with frontline staff across all departments involved to support and guide their process b. Establish linkages and collaboration for primary care transformation concepts (e.g. managed care office; behavioral health department; IT/telecommunications departments, etc.). c. Connecting with fellow CO PCMH facility leads, for knowledge sharing and collective troubleshooting.
4. Prepare for application submission by: a. Facilitating evidence gathering for the NYS PCMH application by completing the PCMH requirements' tracker b. Uploading site-level evidence/documentation to the PCMH SharePoint site.
5. Active participant and collaborator to the Gotham Policy and Procedure Sub-Committees to enforce all required policies are up to date
6. Collaborate with Quality Team on the implementation of the site-level quality improvement efforts toward primary care transformation; identifying areas of improvement and recommending actions for each primary care practice and/or care team.
7. Possess a working knowledge of primary care operations, maintain working relationships with the primary care practice managers/ PCMH Site leads and performs related work as assigned by the executive leadership.
8. Maintain and monitor the PCMH SharePlus, uploading supporting documentations and resources.
9. Travel to Gotham facilities in a regular basis; as well as during mock surveys and or readiness assessment evaluation.
10. Other duties required by regulatory agencies and Gotham Executive team

Minimum Qualifications
1. A Master's degree from an accredited college or university in Public Health, Public Administration, Business Administration, Social Work, Psychology or Rehabilitation Counseling; and
2. Two (2) years of full-time experience in medically-oriented health care and medical support systems environment, one (1) year of which included experience in an administrative or supervisory capacity in such occupations as physical, occupational, inhalation, speech and audiology therapy or as Medical/Laboratory Technologist, Microbiologist (various specialties), Chemist (biochemist), Radiation and X-Ray Technician; or two (2) years of responsible level experience in business management systems or general administration including one (1) year in a supervisory or administrative capacity; or
3. A Baccalaureate degree from an accredited college or university in disciplines listed in "1" above; and
4. Three (3) years of full-time experience, as described in "2" above, including two (2) years of experience in an administrative or supervisory capacity in occupations, as listed in "2" above; or
5. A satisfactory equivalent combination of education, training and/or experience.

Note: Certification or licensure may be required.

Bachelor's Degree, (Master's Degree preferred) from accredited college or university in Health Administration, or approved related program Knowledge and experience in NCQA and NYSDOH PCMH program requirements
A satisfactory equivalent combination of education, training and experience

COMPUTER PROGRAMS/SOFTWARE OPERATED

EPIC and Microsoft Office Applications: excel and PowerPoint is a plus
