Job Description:
- Provide therapy services under the supervision of an Occupational Therapist in a Skilled Nursing Facility setting.
- Work collaboratively with a multidisciplinary team to create and implement individualized treatment plans.
- Assist patients in improving their functional abilities and quality of life.
- Document patient progress and maintain accurate records in a timely manner.
- Communicate effectively with patients, families, and team members.
- Adapt treatment plans as needed to meet the evolving needs of patients.
- Ensure compliance with facility policies, procedures, and regulations.
