Primary Duties:
- Collaborates with Endocrinologists and Nurse Practitioners in developing patient centered diabetes plans.
- Completes patient's diabetes assessment and develops educational goals and a diabetes self-management support care plan based on patients' assessed needs.
- Provides quality diabetes self-management education and medical nutrition therapy to patients and families in individual and group settings. Utilizes appropriate teaching techniques that are sensitive to the learning styles of the patient.
- Follows patients closely (in person and via phone/video conferencing) until the patient meets program goals or is transferred to the virtual control center. May Provide distance visits by video conferencing, telephone, text, and email.
- Partners with MedStar Diabetes Institute and records outcomes in Diabetes Institute database as required.
Qualifications:
- Bachelors degree in dietetics, nutrition required
- Previous pump and CGM experience preferred
- Registered Dietitian current registration required registration eligible status with the American Dietetics Associations Commission of Dietetic Registration.
- Currently licensed in DC or eligible for DC licensure.
- CDE-Certified Diabetes Educator credential preferred/eligible required. If not already CDE, will obtain within 1.5 years required.
