This organizational leadership role is responsible for planning, implementing, and directing activities of the
Patient Safety Program to achieve Zero Harm High-Reliability and meet accreditation standards at AHS. The role
serves as an expert resource to the organization, working with patients, Providers and Caregivers to optimize
patient safety and experience.

Required Certfications:

RN - Registered Nurse with Current Ohio License

Preferred Certifications: Certified Professional Patient Safety (CPPS)-IHI, Certified Professional Healthcare Quality (CPHQ)

Required Experience: 3-5 years Direct Patient Care, Health Care Leadership

Preferred Experience: 5-7 years progressive Health Care Leadership

Leadership Required

Educational Degree: Bachelor's Degree Nursing (BSN)

Preferred Education:
Master's Degree Nursing (MSN) or Master'

Patient Safety Identification and Mitigation of Risk: Effectively report, investigate, and analyze patient safety incidents/adverse events, medical errors and potential risks at AHS.Facilitate incident report analysis through Common Cause Analysis (ex: TJC RCA framework, Apparent Cause Analysis, and other modes of investigation) which results in continuous and sustainable improvement strategies at AHS. Facilitate Failure Mode Effect Analysis (FMEA) to identify and mitigate unintended adverse patient outcomes and evaluate effectiveness of process changes, PRN and annual risk analysis at AHS. Perform Patient Safety Rounds and oversite of AHS Safety Huddle to identify patient safety risks. Empower and Engage Caregivers to identify and participate in resolution of patient safety concerns at AHS. Coordinate disclosure of serious events to patients and/or families in accordance with organizational policy and regulations at AHS with assistance from Risk/Legal and other stakeholders. Assure timely reporting of Incident Reporting to the AHS designated Patient Safety Organization (PSO); Actively participate in PSO continued education sessions. Ensures implementation of best practices, alerts, surveillance and updates to continuously improve patient safety at AHS. Collaborates with Peer Review, Risk Management, Service, Accreditation, and AHS Leadership PRN with Incident Reporting and Common Cause Analysis

Patient Safety Culture Advancement: Champion completion of Culture of Safety Survey at AHS. Facilitate analysis of culture of safety survey results, leading to data-driven action plans lead with targeted outcomes and continuous improvement at AHS. Support and encourage incident reporting throughout AHS through a non-punitive Just Culture event reporting system for all Caregivers and Providers. Collaborates with Engagement and Service Leadership and Committees to promote a High Reliability Zero Harm Culture at AHS. Patient Safety Education: Include patient safety and High reliability in AHS organizational development with New Caregiver Orientation, Adena Essentials, and other Caregiver and Provider orientations PRN (i.e. Zero Harm-High Reliability, Incident Reporting, Safety Culture, Team STEPPS, Life Wings, etc.) Provide ongoing education to leaders, clinicians and staff on the science of safety (high reliability, effective communication, sustaining awareness/alertness) and patient safety objectives at AHS

Patient Safety Strategy Development: Provides leadership and direction to the organizational patient safety strategy at AHS (including Zero Harm, Press Ganey Safety Culture survey, High Reliability, etc.). With appropriable collaboration, directs the development of a comprehensive Patients Safety Plan and updates the plan annualy. Develops internal annual Patient Safety goals (in alignment with NPSG) specific AHS improvement needs.

Patient Safety Daily Operations Management: Manages operational aspects of the Patient Safety Department including ongoing standard's assessments, incident reporting, facilitation of corrective action plans, and design of audits to evaluate effectiveness of improvements. Plans, assigns and reviews work of direct reports to ensure that departmental objectives are met. Hire, train, develop and manage staff to ensure that a qualified staff exists to meet department annual objectives. Recommend and/or develop policies and procedures aimed at enhancing patient safety measures and initiatives, including policies directed toward compliance with recognized patient safety standards. Routinely update and submit the Patient Safety Dashboard and related corrective action plan to department directors and appropriate committees. Responsible for the activities of the Health System Patient Safety Committee and Program.
