Under the direction of the VP, the Director plans, implements and directs Quality Operations and care transitions programs with SBH Ambulatory Care. The Director leads developing action plans to improve care continuity, enhance quality metric performance, and maintaining regulatory compliance. The position holder recommends process improvements to enhance operations and patient care. They assist with audits, Joint Commission preparations, and chart reviews. Additionally, the Director is responsible for managing HEDIS measures within health insurance incentive programs, aiming to monitor, oversee, and improve these measures to enhance care delivery and patient satisfaction. The Director collaborates with stakeholders to drive program excellence and aligns initiatives with SBH health system's goals, promoting innovative and effective patient-centered care.
QUALIFICATIONS:
Education- Master's Degree Preferred, Bachelor's Degree Essential License(s) Required - N/A Experience/Skills Required -
- Provides leadership to the Ambulatory Care Quality & Population Health department, supporting the VP with department strategy, operations and management.
- Directs performance improvement projects for SBH Ambulatory Care Quality Assurance and Performance Improvement (QAPI) Committee.
- Directs the development and implementation of initiatives to improve quality metrics and achieve higher performance in value-based payment contracts.
- Collaborates with clinical and administrative leaders to identify care transition, quality improvement, and growth opportunities, researching and developing solutions.
- Directs special data projects for Ambulatory Care.
- Directs the strategy, design, and implementation of special projects to improve population health.
- Supports the Vice President in implementing new clinical workflows and billable services, ensuring growth in services, volumes and revenues.
- Collaborates with clinical and administrative leaders to identify growth opportunities, and researches and develops proposals & applications.
- Manages and supervises assigned employees / teams, including work assignments, attendance, discipline, performance evaluations and planning requirements.
QUALIFICATIONS:
- Active experience in a health care setting.
- Minimum of three (3) years' experience in progressive leadership roles.
- Knowledgeable in the day-to-day operations in clinic settings.
- Excellent communication, problem solving and writing skills.
- Computer knowledgeable.
- Ability to work with all levels of staff, including administration, physicians, nurses and clerical.
