Salary: $110,000 - $130,000 Annually + $10,000 Sign-On Bonus

We are seeking a Director of Case Management to join our team. This leadership role offers a competitive salary, a $10,000 sign-on bonus, and relocation assistance on a case-by-case basis. As the Director of Case Management, you will oversee all department activities, ensuring compliance with policies, fostering interdisciplinary collaboration, and optimizing clinical resource utilization. This role requires strong leadership, strategic planning, and the ability to engage effectively with both internal teams and external stakeholders.Key Responsibilities:
  • Lead and evaluate departmental operations, including staffing, technology use, and onboarding.
  • Ensure adherence to care coordination and discharge planning processes with proper documentation.
  • Collaborate with CFO and Division leaders to achieve departmental goals.
  • Drive sustainable performance improvement initiatives by analyzing data and evaluating processes.
  • Foster relationships with key stakeholders, including clinical and administrative leadership.
  • Adhere to organizational policies, SOPs, and the company's "Code of Conduct."
Qualifications:

Required:
  • RN licensure with at least 2 years of recent large acute care case management leadership experience.
  • Bachelor's degree in Nursing or related field.

Preferred:
  • Master's degree in Nursing, Health Administration, or Business Administration.
  • Proven experience working in a complex healthcare environment, including engagement with C-suite leaders.
