Essential Functions:
- Develops, implements, and oversees program outreach, participant recruitment, and retention efforts within the community served.
- Responsible for implementing and managing all grant, cooperative agreements, and other contractual arrangements related to the GHHS program.
- Responsible for program management and accountability in accordance with program policies.
- Responsible for oversight and approval of spending, monthly invoices, and abiding by Notice of Grant Award requirements.
- Provide oversight on the establishment of GHHS case management program philosophy, goals, objectives, and organizational policies and procedures on healthcare issues not covered by organization protocols or policies.
- Assures the completion and submission of all reporting according to grant guidelines.
- Oversees the activities of the education sessions, consortium and community events.
- Implement the quality improvement, performance monitoring, and evaluation efforts, modifying the program as appropriate and based on findings
- Tasks to include:
- Provide randomly chosen satisfaction surveys to program participants to ensure services are addressing core competencies and program goals.
- Provide regular review of benchmark progress and core competency focus in services offered.
- Provides written monthly and quarterly project updates
- Lead in data collection for all grant reports and applications.
- Provide monthly performance monitoring results to all case management staff.
- Tasks to include:
- Work with Marketing and Communications Manager to manage all marketing endeavors to promote Healthy Start within the community, including but not limited to assisting with the design of HS brochures, flyers and other promotional pieces.
- Work with staff to identify opportunities to promote Healthy Start within the community.
- Provide direct supervision to all Healthy Start program staff via regular one-on-one supervision, tier supervisions, and group meetings.
- Assess and provide staff trainings specific to core competencies, cultural competencies, and programmatic growth
- Work with Hamilton administration on grant renewal applications
- Maintain consistent communication with HRSA Project Officer and other assigned Healthy Start technical assistance staff
- Other miscellaneous duties as assigned
Requirements:
- Healthcare Knowledge: A fundamental understanding of healthcare services and basic medical terminology.
- Management: At least five (5) years managerial experience working in health or human service field
- Tech-Savvy: Proficiency in basic data entry computer skills.
- Strong Interpersonal Skills: Ability to communicate courteously and effectively with individuals from diverse backgrounds.
- Team Player: An industrious and cooperative nature to foster harmonious working relationships with clinic management, physicians, clinicians, and the public.
- Education: Minimum of Bachelor's Degree is Human Services, Social Work, or related field
Preferred Qualifications:
- Masters Degree
- CPR/First Aid certification
- Experience working with non-profit grant programs and budget management preferred
- Knowledge of The Joint Commission regulations
- Bi-lingual in Spanish, Nepali, French
