MAIN FUNCTION:
The primary purpose of your job position is to plan, organize, develop and direct the overall operation of the Skilled Nursing Facility based on the philosophy of the SNF and St. Joseph's Hospital and in accordance with current Federal, State and local standards, guidelines and regulations that govern our facility, and may be directed by the Administrator, and the Medical Director to ensure that the residents age 18 and over who require skilled long term and short term care including the chronically ill, geriatric and infirmed resident.
DUTIES AND RESPONSIBILITIES
ADMINISTRATIVE / MANAGEMENT FUNCTIONS:
1. Plans, develops, organizes, implements, evaluates and directs all functions and activities
of the nursing staff as well as its programs and activities each shift, seven days/week, in
accordance with current rules, regulations, and guidelines that govern Skilled Nursing Facilities.
2. Develops services / maintains Skilled Nursing philosophy and objectives.
3. Develops and maintains standards of practice for the Skilled Nursing Facility.
4. Develops, maintains, implements and periodically updates Skilled Nursing Policy and
Procedures manual in collaboration with administration and SNF team.
5. Actively involved in SNF QAPI/QA committee with assisting, developing and implementing appropriate plans of action to evaluate and improve the quality of resident care which is cross-disciplinary and cross-departmental.
6. Develops methods for coordination of SNF services to ensure the continuity of the residents' total plan of care.
7. Assists with Development and maintains and periodically updates written job descriptions for each level of nursing personnel which reflects Skilled Nursing Care and activities.
8. Performs administrative duties such as completing medical forms, reports, evaluations,
charting, etc., as necessary.
9. Assists in developing, implementing and periodically updating the written procedures for the admission and discharge planning.
10. Actively participates in survey (inspections) made by authorized government agencies.
11. Makes written and oral reports/recommendations to the Administrator as necessary and/or required, concerning the operation of the Skilled Nursing Facility.
12. Creates a working environment that encourages staff involvement, with improvement of business performance and improvement of resident/family satisfaction.
13. Holds regular communications meetings with staff; continuously coach staff, and support and
encourage training for staff.
NURSING CARE FUNCTION:
14. Participates in the interviewing and selection of residents for admission to the facility and
greets newly admitted residents.
15. Ensures that residents are admitted, transferred and discharged in accordance with current rules, regulations, guidelines and facility policies and procedures.
16. Reports all resident status changes to the Administrator as warranted.
17. Guides clinical coordinators with monthly QAPI projects. May perform QAPI when needed. Plans and holds monthly QAPI meetings.
18. Provides direct nursing care as necessary.
19. Collaborates with other disciplines to maintain a comprehensive, interdisciplinary plan of care..
20. Supervises and coordinates writing the nursing and comprehensive care plan based on resident assessment in accordance with federal and state mandates and the MDS.
21. Ensures timely and accurate completion of MDS and care plan process.
22. Assists with planning and develops training classes to ensure facility growth in overall CMI.
STAFF DEVELOPMENT:
23. Is able to obtain nursing material (policies, procedures or any educational material) that will assist the staff in meeting the day-to-day needs of the resident.
24. Works with nurse educator to develop, implement and maintain an effective orientation program that orients new employees to the facility, its policies and procedures and to his/her job position and duties.
25. Provides leadership training that includes the administrative and supervisory principles essential for charge / staff nurse.
26. Attends and participates in continuing educational programs designed to keep abreast of state and federal mandates for SNF as well as rehabilitation, psychiatric, and gerontological nursing.
PERSONNEL:
27. Assumes responsibility for twenty-four hour scheduling of staff, working with staffing
scheduler as needed following guidelines for staffing.
28. Works with unit coordinators to evaluate work performance of employees, annually and at the
conclusion of probationary period, based on job description. Includes counseling to foster personal and professional growth. Employees' evaluations are completed on time.
29. Manages disciplinary problems as needed through counseling and using accurate,
objective documentation, follows the guidelines of the NYSNA contract, and without regard
to race, color, creed, national origin, age, sex, religion, handicap, or marital status.
30. Participates and / or conducts prospective employee interviews and chooses candidates for hire in coordination with Human Resources. Recommends the number and level of nursing personnel to be employed.
31. Monitors absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times.
32. Reviews complaints and grievances made or filed by SNF Nursing personnel.
BUDGET AND PLANNING:
33. In conjunction with the administrator, responsible for managing the annual Nursing
Department budget.
a) Annual capital equipment requests.
b) Labor and non-labor budgets.
c) Appropriate par level of supplies.
d) Staff correctly uses the charge system.
COMMITTEE FUNCTIONS:
34. Attends and participates in developing the agenda for the Resident Care Conference as necessary.
35. Encourages staff involvement in committee work.
SAFETY AND SECURITY:
36. Monitors nursing personnel to assure they are following established safety regulations in the
use of equipment and supplies.
37. Ensures that the following are maintained in a clean and sanitary manner:
a) Work areas (nurses' stations, medicine prep rooms, etc).
b) Resident care rooms (including treatment areas).
38. Ensures that all nursing personnel follow established departmental policies and procedures to include appropriate dress codes, fire safety, disaster preparedness drills, and infection control.
39. Maintains a procedure for reporting hazardous conditions or equipment.
CARE PLAN AND ASSESSMENT FUNCTIONS:
40. Oversees the development of preliminary and comprehensive assessments of the nursing needs of each resident utilizing the MDS.
41. Assists in development of a written plan of care (initial and comprehensive) for each resident
that identifies the problems/needs of the resident, indicates the care to be given, goals to be
accomplished, and which professional service is responsible for each element of care in
accordance with the MDS+ assessment.
42. Ensures that all personnel involved in providing care to the resident are aware of the care plan and that care plans are used in administering daily care to the resident.
43. Develops and maintains good rapport with all services involved in the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.
RESIDENT'S RIGHTS FUNCTIONS:
44. This position requires you to care for resident's ages 18 plus years. Your approach to residents must reflect knowledge of normal growth and development for the age you are serving.
45. Is responsible for attending all annual mandatory educational programs as required by position.

EDUCATION:
Graduation from an approved school of nursing, with current registration with the New York State Education Department. Bachelor's Degree in Nursing required; a Master's Degree is desirable. Active membership in the professional organization is recommended.
EXPERIENCE:
A minimum of five years experience as a staff nurse with at least two years experience in a supervisory capacity in a long term care facility.Working knowledge of the facility Philosophy, organization, and functions of all departments, pertinent policies, regulations and procedures of the facility.
CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS:
Basic Life Support (BLS) required within 90 days of hire date.
PHYSICAL DEMANDS:
Good physical and mental health is required to allow sustained attention to many details over extended periods. Visual and aural acuity required to detect changes in patient's conditions.
