Summary of Duties and Responsibilities:
Leads the population health team and is responsible for the development, implementation,
and evaluation of all Value-Based Programs, such as ACO REACH and Medicaid Managed Care. Works with the medical and clinical operations team to develop care teams to succeed in Value-Based Care, and leads programs, initiatives, and projects and ensures their successful implementation and operation. Must continuously monitor the goals and objectives of the Population Health initiatives while maintaining alignment with the strategic goals and objectives of Variety Care. Works closely with a variety of stakeholders, coordinating the activities across the organization to develop, implement, and evaluate our Population Health programs and initiatives.
Primary Duties and Responsibilities:
Directs and leads population health programs, initiatives, and projects and ensure their successful operations to support the organization's mission and Strategic Plan Leads the team to identify priority health needs, gaps in services, and areas for enhancement and development of a Value-Based Care model. Introduces evidence or practice-based health promotion and wellness programs to address priority community and healthcare needs. Assesses staff's training & technical assistance needs and facilitate/coordinate training and technical assistance needed to ensure staff's maximal performance. Assures that Variety Care's leadership and other executive and management teams receive timely, relevant information to assess progress and evaluate the performance of programs. Enhances community engagement by recruiting community partners and collaborating with them to improve population health. Directs and manages the operations of Care Managers, Community Health Workers, and any other members of the Population Health Team. Collaborates with VPMA, CMO, CCO, CQO, and other executive team members to develop key performance indicators (KPI) for grants and programs. Work with Value-Based partners to ensure maximum in improved patient outcomes and shared savings (i.e., MHN and ACO REACH, Managed Medicaid awardees, etc.). Collaborate with the CQO and/or grant manager to design data collection, analysis, reporting tools, and dashboards, and regularly assess the data collection tools, data repository, and reporting templates for maximum efficiency. Manages administrative tasks such as employees' performance assessments, compliance with annual in-service training and documentation of training, certifications, time, and attendance tracking and reporting, time off and vacations, certifications, etc. Develops and maintains strong partnerships across key units across Variety Care clinics and in the community. Builds interdepartmental and interdisciplinary relationships to assure collaboration and effective/efficient operations both within the organization as well as in the community. Ensures the programs are aligned with the standards of the patient-centered medical home care model and fosters the administration of evidence-based healthcare in a new and innovative way. Remains current with new trends and best practices and incorporates into Population Health programs. Leads and/or serves on a variety of appropriate internal and external committees to represent Population Health. Adheres to Variety Care policies and procedures. Leads and/or serves on a variety of appropriate internal and external committees to represent Variety Care and Population Health within the organization. Reviews and analyzes population and patient data related to clinical care and outcomes to evaluate performance, reveal trends, and identify opportunities for improvement. Prepares various reports to present findings and to highlight successes and opportunities for improvement or intervention. Supports Variety Care's accreditation as a Patient Centered Medical Home and our commitment to provide care to all Variety patients that is Safe, Effective, Patient Centered, Timely, Efficient, and Equitable. Provide leadership and work with all staff to achieve the goals of the "Quintuple Aim" of healthcare reform-to improve the experience of care, improve health outcomes, decrease healthcare costs, improve provider experience at work, and ensure equity. Requirements, Special Skills or Knowledge:
Bachelor of Science in Nursing (BSN). Active RN License in the State of Oklahoma. At least two years of clinical nursing experience. At least two years of supervisory experience. Basic Life Support (BLS) for the Health Care Provider certification. Proficient computer skills in Microsoft Office, Outlook, and Excel.
Preferred Requirements, Special Skills or Knowledge:
Bilingual (English/Spanish). Knowledge of EPIC.
