The Director of Population Health provides competency in Population Health, Business Analysis, and in Quality Assurance & Improvement review.

ESSENTIAL JOB FUNCTIONS
  • Provides expertise in defining, collecting, analyzing and providing operational recommendations for performance management based on healthcare information.
  • Demonstrates competence at using analytic tools to uncover trends and significant findings in analyzed data to understand the complex needs of clients and systems of care.
  • Conceptualizes and articulates an organizational understanding of health care data at the individual served, program, and system level, particularly how it impacts performance management and revenue cycle maximization.
  • Utilizes expertise in facilitating, defining, and documenting report and information needs and acts as liaison between program and IS/IT staff.
  • Provides supervision to Quality Supervisors and Quality Analysts on best practices on data collection, reporting, and analysis of data needs.
  • Partners with Executive Leadership to define quality structure and staff roles within departments.
  • Utilizes expertise at facilitating, defining and documenting various business and clinical workflows and using this information to improve data collection and efficacy.
  • Provides expertise in the use of the Electronic Health Record (EHR) for documentation and reporting purposes.
  • Understands and utilizes project management skills in managing multiple, concurrent projects and people.
  • Organizes, convenes and oversees the BHN Quality Committee, focused on data, performance management, and improvement.
  • Supervises staff who conduct audits of charts using criteria established by the organization.
  • Assures that BHN is meeting all targets related to Value Based Payment (VBP) agreements contained in contracts.
  • In collaboration with program management staff, develops a Quality Remediation Plan (QRP) in response to identified findings of non-compliance with organization/regulatory standards or performance metrics.
  • Collaborates with Clinical Director to identify trends and implement solutions to enhancing the quality of care delivery.
  • Oversees the development of, and compliance with, BHN's Performance Improvement Plan for CARF.


Requirements

  • Minimum of Bachelor's Degree in a related field, Master's Degree preferred.
  • Minimum of 5 years' experience in the human services or related fields.
  • Minimum of 5 years' of conducting healthcare analysis activities.
  • Experience in engaging technical and non-technical staff in the collection and use of data required.
  • Must be committed to consumer empowerment, growth, human rights, and community integration.
  • Must have a working knowledge of regulations.
  • Must have familiarity with word processing, spreadsheets, and email systems.
  • Must understand the importance of information to inform system change.
  • Excellent verbal and written communication skills.
  • Must be detail-oriented.
  • Valid driver's license and a reliable vehicle available for all working hours required.
