Job Overview

Provides direction, oversight, and coordination of services and resources to ensure the safe and effective provision of nursing care across the primary care or specialty care service lines in collaboration with service line leadership. Consults and educates staff and other health care professionals as an expert in nursing and allied health care in an ambulatory setting. In addition, this individual establishes, and monitors standards of care related to primary care or specialty care nursing and allied health practice and participates in research and quality improvement activities while providing leadership in creating and promoting an environment of evidence-based practice and interdisciplinary care delivery.

Directs initiatives that result in improved clinical systems, processes and/or patient outcomes which includes the following:

Oversee and coordinate the management of staff within ambulatory care resource support teams.

Guides staff in applying and implementing research and regulatory guidelines to practice, resulting in improved patient care/outcomes.

Provides leadership by assisting staff in developing and documenting an interdisciplinary plan of care that assists others in utilizing current theory in care delivery.

Assists staff in integrating standards of proactive, care guidelines, research and regulatory findings and technological advances into clinical practice.

Collaborates with executive leadership, clinic administration and clinical site leads on care initiatives that improve quality, efficiency and patient experience which includes the following:

Coordinate with the Executive and Operational Leadership teams to identify activities and projects that advance the care agenda for the organization.

Partner with system leaders to align work with system nursing objectives.

Ensures compliance with all state and federal guidelines in the practice of nursing.

Leads clinical aspects of continuous quality improvement efforts which includes the following:

Participates with interdisciplinary work groups by providing clinical leadership that promotes effective, deliberate, and integrated system changes.

Participates in quality improvement efforts to achieve expected levels of patient satisfaction, clinical outcomes, and cost effectiveness.

Provides expert clinical and coordination of services through the following:

Ensures that contemporary standards, new research findings and technological advances are integrated into nursing practice and allied health practice in the clinics.

Ensures that advanced nursing proactive concepts are consistently demonstrated.

Expertise and advanced practice concepts are demonstrated through expert care, consultation, or education for select patients/families/customer.

Acts as an advocate for area of expertise.

May manage a centralized team (i.e. diabetes educators, medication refills)

Establishes and monitors standards of care related to Primary Care or Specialty Care in collaboration with nursing managers, staff, and interdisciplinary staff which includes the following:

Takes action to integrate best practices regarding national and community standards into Fairview standards, care guidelines, policies, and procedures.

Consults and collaborates with internal/external customers to promote expert clinical care in population-based area of specification: e.g., Clinic leadership, Ancillary Services, Quality Improvement and Education teams, Medical Staff, and community.

Consults and collaborates with staff to facilitate complex clinical problem solving for clients in specialty population by utilizing a variety of resources.

Serves as a representative for primary care/specialty care on policy and regulatory committees.

Leads training and education team to provide education to nursing staff, other interdisciplinary staff, students, and faculty to improve their skills and improve patient care (in collaboration with organizational learning specialists, nursing practice and education and clinic leadership). The key components of this job expectation include the following:

Learning activities are planned to meet the needs of diverse work force, with different learning styles, and within a changing health care environment.

Ensures competencies are developed, implemented, and consistently updated for all specialty care clinical staff.

Current practice, new research findings and technological advances are integrated into the educational design.

Fosters a climate that promotes open communication, learning and professional growth.

Creates a learning environment, which fosters critical thinking, technical and interpersonal skill development.

Learners assume responsibility for their own learning outcomes and receive timely and appropriate feedback during and after educational activity.

Provides educational opportunities (competency assessment, clinical supervision, specialty core curriculum courses, action plans, performance improvement classes) for a diverse group of learners, including nurses, medical staff, and other healthcare professionals.

Participates in nursing research, publication as indicated. Communicates research knowledge and findings to improve clinical practice and includes the following expectations:

Researchable clinical problems are identified.

Research of staff, students and faculty is facilitated.

Research findings are incorporated in CN's clinical proactive.

Staff, students, and faculty are assisted in the research utilization process, integrating clinical research findings into nursing proactive.

Consistently demonstrates positive example for personal and professional development which includes the following expectations:

Participates in professional organizations and ongoing educational activities related to clinical knowledge and professional issues.

Knowledge and expertise is demonstrated through certification, publication, presentation and/or research activities.

Models' professional growth through an individual development plan, based on lifelong learning and accomplishment of annual goals relevant to professional development and job performance.

Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.

Qualifications:

Required

Education
  • Bachelor's degree in nursing

Experience
  • 7-10 years nursing, clinic management or healthcare experience. Experience in nursing and allied health care of ambulatory care patients.

License/Certification/Registration
  • RN license State of MN


Preferred

Education
  • Master's degree in nursing, health care administration, business administration or related field. If masters' degree is not in nursing, then bachelor's degree must be in nursing.

Experience
  • Leadership in an ambulatory care setting

License/Certification/Registration
  • ANCC advanced certifications

Compensation Disclaimer

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
