Miami Jewish Health is one of the largest providers of healthcare and living options for aging adults in the Southeast. We operate Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, the largest PACE program in Florida, which serves as health plan, healthcare provider, and social center designed for older adults with complex medical needs. We have four adult day health centers that serve participants in Miami-Dade and Broward counties.
Job Title: Director Social Services
Job Summary
The Director of Social Services provides visionary leadership and strategic oversight for the Social Services departments across all PACE centers within the organization. This role ensures the standardization, compliance, and continuous improvement of psychosocial care and support services delivered to participants. As a corporate leader, the Director collaborates across departments-including Intake, Clinical, and Quality-to align social services with organizational goals, regulatory requirements, and participant-centered care principles.
Essential Job Functions
Serve as the top-level authority for Social Services across all PACE locations, ensuring operational consistency and high-quality psychosocial care.
Develop and implement system-wide policies, procedures, and standards for social work practices in alignment with federal, state, and accreditation guidelines.
Guide site-level Social Workers (BSW, MSW, LCSW) in the execution of their duties and ensure they are adequately trained, supported, and evaluated.
Lead the development of strategic goals and benchmarks for the department, tracking outcomes and driving performance improvement across all centers.
Conduct regular audits, site visits, and compliance checks to ensure adherence to internal standards and external regulatory expectations.
Oversee the integration of social workers into eligibility evaluations, including support in determining participants' ability to remain safely in the community and facilitating coordination with state agencies for level of care certification.
Ensure social services play a proactive role in educating prospective participants and their families about program benefits, expectations, and enrollment obligations.
Partner with Finance and Eligibility teams to support financial counseling and Medicaid-related processes for participants during intake and re-certifications.
Champion a participant-centered approach by ensuring social work services are fully integrated into the Interdisciplinary Team (IDT) model.
Guide and support implementation of behavioral health initiatives, trauma-informed care, grief support, caregiver counseling, and crisis intervention strategies.
Promote best practices in managing social determinants of health, elder abuse prevention, and community resource coordination.
Monitor departmental performance indicators and lead data-driven quality improvement initiatives.
Serve as the primary point of contact during audits and regulatory reviews related to social work functions.
Ensure all documentation, assessments, and care planning activities are completed in compliance with 42 CFR 460 and CMS/PACE guidelines.
Job Requirements
Education
Master's degree in social work (MSW) required; LCSW licensure strongly preferred.
Experience
Minimum 7 years of progressive leadership experience in healthcare or senior care, including multi-site oversight of social services teams
Abilities Required
Knowledge of PACE model of care and regulatory requirements.
Demonstrated success in standardizing practices across multiple sites and managing complex workflows involving multiple stakeholders.
