Job Summary:

Responsible for the ongoing operation of the ECM hub. The goal of the program is to provide physician managed triage of patient's acute care needs, innovative same day care delivery, and to provide expert medical knowledge and medical information to best navigate and access our complex care delivery system. These functions combine to ensure our members receive the highest quality care and our physicians receive the best decision support in the most efficient and effective manner. Previous experience in a hospital based setting is preferred.

Major Responsibilities/Essential Functions:
  • Accountable for the performing the duties of the ECM hub. The ECM hub provides the following services:
    • Repatriation of medically stable patients from non-core Emergency Departments (ED's) and Inpatient status to KP core hospital.
    • Coordination of care including timely venue management for patients at home, Medical Office Building, Acute Care Center (ACC), Clinical Decision Unit (CDU) and Skilled Nursing Facilities who require further acute care services in a new venue of either higher or lower acuity.
    • Provides consultation for PMG MDs (Primary and Specialty Care) to develop the ideal plan and place of care for our patients.
    • Physician medical advice and triage recommendations to the advice nurses as needed to determine the best disposition for members
    • Support of ECM hub staff in determining medical necessity for same day ambulance transfer.
    • Provides clinical information to physicians caring for our members at non-core facilities.
    • New member onboarding prescription review and medication ordering.
    • Supports Home Health initiatives.
  • The Hub Physician must have an expert understanding of our care delivery system and our contractual arrangements in order to provide the most effective care coordination and support.
  • Maintain an accurate understanding of all pertinent EMTALA and other compliance and regulatory issues related to patient transfer.
  • Keep abreast of changes in clinical practice that impact the need for inpatient admission and/or other levels of care.
  • Collaborates with ECM Hub Chief and management of non-physician staff to provide performance feedback to staff.
  • Shares best practices and learnings with each other and staff as appropriate.
  • The Hub Physician appropriately documents necessary clinical information including plan of care in KPHC.
  • Complies with regional policies, including clinical guideline standards, human resources policies, compliance and general administrative.
  • Provides effective leadership and communications within the department.
  • Personally, models high standards of clinical and personal behavior, a positive attitude and excellent clinical performance.
  • Works collaboratively with team members to realize organization's mission and goals for a consistent application of work processes and clinical policies.
  • Collaborates with Inpatient Quality Management (IQM) to ensure the most appropriate resources are utilized for our patients.
  • Maintains knowledge of best practices for effective ambulatory and inpatient care as well as community standards of care and practice changes especially in the area of urgent care or emergency medicine.
  • Ensures that access to protected health information (PHI) will be limited to the minimum necessary required to effectively perform job functions. Assures compliance with applicable federal, state and local laws and regulations. Complies with the principles of Responsibility, the Code of Conduct for KP, internal policies and procedures, professional standards and accreditation standards.
  • May perform additional duties as necessary.
