Day Kimball Health is hiring for an Advanced Practice Provider to join our Endocrinology practice!

Position Summary:

Join an outpatient based, multi-group practice in Northeast Connecticut that is seeking an Advance Practice Provider to join our Endocrinology practice. You will be teamed with a Board-Certified Endocrinologist to support the rapid growth of our program.

Endocrinology Advanced Practice Provider (APP) - Job Responsibilities:
  • Provide comprehensive care in general endocrinology, with a strong focus on diabetes management, insulin pump technology, osteoporosis, and thyroid disorders.
  • Conduct patient assessments, develop treatment plans, and manage chronic endocrine conditions in collaboration with physician.
  • Utilize bedside ultrasound for diagnostic and procedural applications.
  • Interpret and act upon lab and radiology findings, with on-site services available for streamlined patient care.
  • Educate patients on disease management, lifestyle modifications, and treatment adherence.
  • Prescribe and adjust medications, including insulin therapy and hormone treatments, based on patient needs.
  • Coordinate with multidisciplinary teams to ensure integrated and patient-centered care.
  • Monitor and manage patients using CSMS insulin pump technology.
  • Stay current with advancements in endocrinology and participate in continuing education and training.
  • Maintain accurate and timely documentation in compliance with healthcare regulations and best practices.


Endocrinology Advanced Practice Provider (APP) - Education and Experience Requirements:
  • Master's degree from an accredited Advanced Practice Provider Program with focus on relevant specialty.
  • National Certification as an Advanced Practice Provider.
  • Active Connecticut State Licensure as an Advanced Practice Provider.
  • 2-3 years' experience in a clinical setting providing direct patient care preferred.
  • Experience in endocrinology strongly preferred.
  • Training or certification in diabetes management, insulin pump therapy, and osteoporosis treatment strongly preferred.


Endocrinology Advanced Practice Provider (APP) - Knowledge, Skills, and Abilities:
  • Strong understanding of endocrine disorders, including diabetes, thyroid disease, osteoporosis, and adrenal and pituitary conditions.
  • Proficiency in managing insulin therapy, including CSMS insulin pump technology and continuous glucose monitoring systems.
  • Ability to interpret lab results, imaging studies, and conduct bedside ultrasounds for endocrine-related assessments.
  • Skilled in counseling patients on disease management, lifestyle modifications, and medication adherence.
  • Ability to work effectively within a multidisciplinary team, coordinating with physicians, dietitians, and other healthcare professionals.
  • Strong analytical and problem-solving skills to assess, diagnose, and manage complex endocrine conditions.
  • Knowledge of healthcare regulations, documentation standards, and best practices in endocrinology.
  • Familiarity with electronic health records (EHR), insulin pump software, and other relevant medical technologies.
  • Excellent verbal and written communication to effectively interact with patients, families, and healthcare teams.
  • Ability to stay updated with advancements in endocrinology and integrate new treatment protocols into practice.
