This is a full-time primary care role providing patient care in a post-acute care setting in a skilled nursing facility (SNF) located in the Watertown, New York, area. You will round 5 days a week with the flexibility to choose your rounding schedule.

The neighborhood parks form the cornerstone of the City's park system and include facilities such as The Veteran's River Walk, Waterworks Park, Bicentennial Park, and Marble Street Park are all located along the magnificent Black River providing scenic views, trails, and picnic areas. Watertown's historic Thompson Park serves as a central gathering place for a variety of community events. The Park is an Olmstead design and is home to many scenic trails. The park is also home to the NYS Zoo where one can see a collection of animals that are native to the area.

Job Duties
- Primary care rounding in skilled nursing facilities
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Collaborate with clinicians, facility leaders and others regarding care and services
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Provide oversight, guidance, and mentorship to our nurse practitioners
- Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
- Represent TeamHealth on appropriate external levels engaging and establishing/maintaining relationships within the post-acute care community
- Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues
Qualifications
- Minimum of 1 year primary care experience
- Post-acute care experience a plus
- Active and unrestricted New York license or application in process
- Active and unrestricted DEA/prescriptive authority or application in process
