Description:
Under the physician's orders, a Nurse Practitioner (NP) is responsible for performing Face-to-Face encounters. The documentation from the Nurse Practitioner encounter is utilized by the Medical Director to determine certification into a sixty (60) day hospice benefit period. The visits are scheduled by the Administrator to be performed within thirty (30) days before the initial start date of any sixty (60) benefit period. The Nurse Practitioner's schedule is coordinated by the Administrator/DON. The Corporate Medical Director serves as a consultant.
QUALIFICATIONS:
1. Graduate of an accredited school of nursing
2. Must be currently licensed (in good standing) in the state where the Hospice Program is located.
3. Documentation of good physical and emotional health.
4. Speaks, writes, reads and comprehends English.
RESPONSIBILITIES (including but not limited to):
1. Fulfills duty of per diem NP functions as assigned.
2. Complies with accepted ethical conduct and professional Standards of Nursing Practice as set forth by the American Nurses Association for Advance Practice.
3. Assess patient's condition and family needs through the collection of health data and an encounter with recognition of physical, psychological and emotional needs related to decline based upon the specific terminal diagnosis.
4. Completes the Nurse Practitioner Attestation of Face-to-Face Encounter with the certification dates reflected, and the actual date the face-to-face took place with a brief narrative of those findings. The Nurse Practitioner will print their name as authentication of their signature. The face-to-face encounter may not take place more than thirty (30) day before the initial start date of the 60 day benefit period.
5. Works with the case managing RN and the Medical Director team to collect necessary comparative data to support the determination of decline in status.
6. Completes all forms, records, charts etc. according to office policy and procedure.
7. Checks with the Administrator weekly for schedule and activities needed.
8. Accepts accountability for own practice.
9. Maintains competency by participating in training and education programs.
PHYSICAL DEMANDS:
Able to lift, push, and pull light to moderate loads. Moderately heavy physical effort. May occasionally do heavy lifting, pushing, or pulling.
LICENSES and CERTIFICATIONS:
1. Licensed as a Nurse Practitioner
2. Current driver's license
3. Current Automobile Liability Insurance according to Policy & Procedure
