Job Description:

- Work Monday through Friday with a shift starting at 08:00 AM and ending at 04:30 PM.
- Participate in a required weekend rotation, which may occur every other weekend or every third weekend.
- Conduct discharge planning for patients in medical/surgical units, maternity health, and intensive care units.
- Collaborate with healthcare teams to ensure smooth transition and continuity of care post-discharge.
- Use knowledge of discharge planning to contribute effectively to patient care management.
