GENERAL SUMMARY: The registered nurse (RN) demonstrates key behaviors in the areas of care delivery, leadership, collaboration, and professional growth. Care delivery includes assessing status, identifying problems, developing, and implementing a plan of care through the delivery and delegation of care, evaluating care delivery, promoting patient/family education and involving the patient/family and other members of the health care team in planning and implementation to meet expected healthcare outcomes. Care is delivered, viewing the person as a whole being, including but not limited to physiological, psychological, and spiritual aspects.
Consideration of the individual's age, developmental level, learning ability, health status and belief system are integral in care delivery. Leadership is evidenced as RNs influence individuals or groups to accomplish goals through guiding, directing, teaching, coaching and motivating others as well as delegating duties to other licensed and non-licensed personnel. Communication and collaboration with patients, families, physicians, and other health care professionals/disciplines is essential in delivering care.
Professional growth is essential in providing care, as the RN shares knowledge, gains knowledge and contributes to knowledge through participation in formal and informal continuing education programs and research. Follows attendance standards and expectations: has minimum tardiness or absenteeism 2. Completes all hospital wide mandatory educational programs. Participates in continuing education activities. Completes all orientation requirements and departmental hemodialysis competency testing requirements annually in a timely manner 3. Responds appropriately in emergency situations 4. Adheres to hospital policies and procedures as well as department specific policies and procedures: examples of hospital wide policies include infection control, restraints, and safety policies and procedures. Participates in hospital and unit quality initiatives 5. Adapts to changing patient assignments as volume and acuity fluctuate 6. Manages physical resources (supplies & equipment) appropriately, assuring that adequate supplies and equipment are available and in good working order. Reports machine issues to supervisor/charge nurse.
Puts in tickets through the equipment management portal/or calls for emergency service to appropriate vendor and tags machines out of service as necessary. Performs regular equipment quality assurance, monitoring, disinfection, maintenance and Performance Rating repairs of dialysis equipment and water purification system. Troubleshoot malfunctions as needed. Escalate immediate safety concerns to manager and medical director as necessary 7. Operates equipment safely and accurately 8. Documents accurately, in detail, timely, legibly and appropriately: all hemodialysis flowsheets, treatment delays, interventions, evaluations, education, referrals (as appropriate) and current status. Accurately and promptly implements physicians' orders and alerts charge nurse/Dialysis supervisor RN and Nephrologist any concerns, treatment delays or issues 9. Assesses, plans, organizes, implements, directs, evaluates, and revises care delivery to individuals and groups of patients in a timely manner; bases this on the health status, standards of care, age and developmental level, and language skills of the patient(s) 10. Communicates pertinent information regarding hemodialysis patients to assigned RN and/or Renal physician/Hemodialysis RN Supervisor as needed 11. Implements the plan of care in a timely manner; based on patient assessment, need and ability; collaboration with others as indicated; and desired outcomes 12. Delegates to licensed, unlicensed and ancillary staff appropriately, considering their skills and experience and the needs of the patient when making assignment. Provides support and guidance to staff as appropriate 13. Interprets patient data accurately, and utilizes that data independently or in consultation with other health team members to plan and revise care 14.
Safely secures, administers, and controls hemodialysis medications, including intravenous solutions; has skills to perform IV venipuncture, maintains IV lines (CVP/PICC) dialysis catheters, Fistulas, and grafts 15. Evaluates the patient's response to hemodialysis and any hemodialysis ordered medications given during treatment, provides necessary follow-up 16. Educates patients/significant others, family members, coworkers and physicians while considering identified learning needs, developmental levels, language skills, functional capacity and learning styles of the individuals or groups being educated. Educates patients regarding the Hemodialysis procedure. Explains aseptic procedures to patients caring for catheters to avoid infection 17. Utilizes restraint alternatives and restraints according to policy and procedure as applicable when in acute treatment area 18. Educates parent/guardian/caregiver/patient based on age and learning needs of patient, the importance of hemodialysis: lifestyle changes, dietary restrictions, care of dialysis ports/lines 19. Possesses knowledge to appropriately monitor, apply and properly use special equipment for hemodialysis care and treatment as applicable 20. Creates a working climate that provides growth and job satisfaction for self and others: works well with others: is respectful to all; maintains confidentiality; communicates with others in a professional manner and tone; participates in department meetings 21.
Participates in problem solving activities to address process issues, personnel issues and performance issues; utilizes chain of command for concerns/issues, communicates concerns to the appropriate person in a professional manner and tone; works proactively to avoid problems 22. Participates in orientation and in-service training of personnel, as indicated 23. Assists in reviewing and revising policies and procedures, as indicated 24. Performs phlebotomy procedures related to blood draws from dialysis access ports/lines. Performs water cultures, machine disinfection as required. Report quality initiatives per our current performance improvement monitor 25. Institutes Hemodialysis treatment per policy and based on physician's order 26. Orders and maintains inventory of expendable supplies for Hemodialysis 27. Other duties as assigned KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: Education and/or licensure requirements: Graduate of an accredited school of nursing Active Illinois RN license. BLS required at time of hire Experience: 1 year of hemodialysis experience strongly preferred Interpersonal Skills: Effective communication skills with patients' families/significant others, physicians and coworkers
The level of knowledge normally acquired thorough graduation of an accredited school of nursing and licensure as an RN in the state of Illinois Demonstrates knowledge of the principles of aging, physical limitations, age associated illnesses, education and unique psychological needs of patients who are 16 and older Working Conditions: Work is performed in a typical hospital lobby environment. Work requires minimal physical demands of standing, walking, bending, lifting, and stooping.
