Job Description

Purpose:

Under the supervision of the Director of Pharmacy and the Lead Antibiotic Stewardship Coordinator, performs assigned duties involving any of the following:
Reviewing medication orders and laboratory results, confirming therapy with prescribers, acting as a change agent to ensure optimal care and optimal patient outcomes. Performs duties in a professional manner with varying degrees of latitude utilizing independent initiative and professional judgement that would ensure patient safety and positive outcomes.

Specific Duties and Responsibilities:
Displays knowledge of appropriate drug therapy related to clinical expertise.
Reviews all antibiotic and antiretroviral medication orders for appropriateness of therapy, which includes, recommendations for appropriate IV to PO switch, review appropriateness of schedule, dosing and length of antibiotic therapy, recommend therapy and dosing adjustments with regards to applicable patient demographic information and laboratory results.
Performs appropriate drug information research requested by the medical, nursing and pharmacy staff and provides relevant responses in a timely manner to ensure uninterrupted treatment to patients.
If requested, in collaboration with Medial Staff of the Hepatitis C Clinic and Infectious Disease Clinics will provide comprehensive counseling to patients who have been diagnosed with either Hepatitis C or HIV. The counseling will include but not limited to initiation or rational for therapy, review of side effects and adverse events of medication used in the patient's treatment. Ways for the patient to manage expected side effects related to therapy, the importance of adherence to therapy.
Participate in daily rounds with the Infectious Disease physician s and or general Medical rounds.
Provide educational presentations and in-services to the medical, nursing and pharmacy staff.
Contribute to the Hospital Pharmacy Newsletter.
Develop and implement cost saving initiatives.
On a daily basis, monitor and review positive blood cultures reported in the microbiology lab.
Participates in inter-departmental activities, committees, meetings and other functions as required.

Minimum Qualifications
1. Possession of a valid NY State license to practice pharmacy.
2. Two years of post licensure experience; or
3. Completion of a post graduate specialty residency or fellowship program; or
4. Current Pharmacy Board Certification.
