Job Description

Oncology Clinical Pharmacy Responsibilities
Participate in patient care rounds as a part of the inpatient Hematology/Oncology consult team
Collaborate with the care team to optimize medication therapies to prevent and reduce medication errors and adverse effects
Monitor treatment regimens to ensure appropriateness of use, dose, dosage form, and drug interactions
Provide pharmacokinetic consultations as needed
Provide patient specific education and counseling on chemotherapeutic regimens as needed
Provide medication information to interdisciplinary Oncology teams
Facilitate, develop, and implement lectures to pharmacy staff in oncology related topics
Support the Pharmacy staff in the clinical care of Oncology patients.
Provide in-services to medical staff on oncology related pharmacological topics
Provide in-services to nursing staff on oncology related pharmacological topics
Develop medication guidelines, order sets, and protocols in collaboration with clinical staff
Pursue opportunities for optimization of Electronic Medical Record
Document pharmacotherapy interventions within the Electronic Medical Record
Provide oversight in the approval and use of nonformulary medications in the Oncology Infusion Center

B. General Clinical Pharmacy Responsibilities:
Monitor drug therapy regimens for allergies, contraindications, interactions, adverse drug reactions, appropriateness of indication and dose, and therapeutic duplications.
Suggest appropriate, cost-effective therapeutic alternatives to non-formulary medications as needed.
Coordinate the selection and safe use of alternative medications in response to medication shortages.
Facilitate patient transitions of care as it relates to medication management.
Establish productive and collaborative relationships with pharmacy staff and other disciplines.
Provide clinical pharmacy consultations for the multidisciplinary healthcare team.
Provide responses to drug information requests to the multidisciplinary healthcare team.
Facilitate the development and implementation of treatment guidelines, protocols, formulary changes as approved by appropriate committees.
Review the use of targeted medications to ensure safe, effective, and economical use and report results to the Pharmacy and Therapeutics Committee to promote process changes and optimize medication utilization.
Prevent, detect, monitor, document and report adverse drug reactions and medication errors.
Implement strategies to maximize cost-effectiveness of high cost medications.
Prepare, review and present drug use evaluation reports to various committees (e.g. antibiotic stewardship reports, medication event reports, pharmacy interventions, etc.)
Document clinical interventions in the electronic medical record.
Collect documentation and analyze data related to clinical activities.
Coordinate policies and procedures for clinical pharmacy activities (e.g. therapeutic interchange, unacceptable abbreviation, look-alike/sound-alike drugs, medication events reporting, etc.).
Participate in quality assurance and performance improvement activities.
Maintain proficiency in the operations of the pharmacy department and perform all duties of the pharmacy staff as required.
Complete special projects as assigned by supervisors or pharmacy management.
Support the pharmacy department's clinical endeavors.
Contribute to the publishing of the pharmacy newsletter.
Supervise and conduct continued education to the pharmacy staff.
Participate in hospital educational programs.
Precept and mentor pharmacy students and residents.
Sustain professional competence and development through participation in workshops, seminars, and professional organizations.

Minimum Qualifications

For Level I:
1. Possession of a valid NY State license to practice pharmacy.

For Level II: In addition to meeting the qualification requirements of the lower level:
1. Two years of post licensure experience; or
2. Completion of a post graduate specialty residency or fellowship program; or
3. Current Pharmacy Board Certification.

For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:
1. One year of post licensure supervisory experience or specialty clinical experience.
