Minimum Qualifications
For Level I:
Possession of a valid NY State license to practice pharmacy.
For Level II:
In addition to meeting the qualification requirements of the lower level:
Two years of post licensure experience; or Completion of a post graduate specialty residency or fellowship program; or Current Pharmacy Board Certification.
For Level III:
In addition to meeting the qualification requirements of the lower levels, an additional:
One year of post licensure supervisory experience or specialty clinical experience.

Job Description
PURPOSE OF POSITION
- To promote leadership commitment to appropriate pain management and opioid stewardship
- To participate in transitions of care program and conduct quality discharge counseling/discharge checklist
- To develop, implement, and maintain organizational policies that support evidence-based approaches to multi-modal pain management.
- To facilitate collaborative and multidisciplinary performance improvement activities where appropriate
- To track, monitor, and report performance data on opioid prescribing, patient-reported outcomes, and adverse events in order to identify opportunities for improvement and assesses the impact of opioid stewardship efforts
- To monitors opioid stewardship compliance with accreditation standards and internal policies/procedures
- To makes recommendations for quality improvement activities, process improvements, training and/or education when concerns are identified
- To report any identified quality of care issues pertaining to individual patients or staff to the appropriate medical staff committee or clinical leadership
- To establish accountability for creating, promoting, practicing, and maintaining a culture of opioid stewardship
- To participate in quality management initiatives such as, but not limited to: medication use evaluations, adverse drug reaction and medication error reporting, clinical interventions, and inventory management documentation and reporting.
AREAS OF RESPONSIBILITIES
- Develop/revise pain management clinical care guidelines that address opioid/analgesic therapies for acute and chronic pain
- Educate pharmacy and other clinical staff on opioid/analgesic therapy
- Assist to achieve full compliance with The Joint commission released pain assessment and management standards such as
- Promoting safe opioid use by identifying high-risk patients
- Monitoring high-risk patients
- Conducting performance improvement activities focusing on pain assessment and management to increase safety and quality for patients
- Identify patients who may require prescriptions for reversal agents
- Monitor of inpatient orders without respiratory monitoring
- Naloxone administrations for reversal of respiratory depression
- Ensure medication errors and ADRs are reported and pathways developed to prevent future occurrences
- Support of quality improvement projects and research initiatives
- Performance of medication use reviews to assess cost effectiveness and medication appropriateness
SUMMARY OF DUTIES AND RESPONSIBILITIES
- Responsible for developing, implementing, and leading an opioid stewardship program
- Work with other health care providers in evaluating and implementing best practices related to opioid prescribing as well as developing metrics to measure safe and effective opioid/analgesic management in the hospital and clinic settings
- Provide cost-effective pharmaceutical care to patients receiving opioids and other pain management medications
- Optimize cost savings and cost avoidance via formulary adherence and best practice clinical guidelines.
- Develop and conduct target drug programs and medication use evaluations as needed and report results to the Pharmacy and Therapeutics Committee.
- Develop clinical guidelines and protocols for the institution
- Provide highly specialized education and training support to other professional services in the broad area of advanced pharmacotherapy
- Precept Pharm.D. Clerkship students on the Internal Medicine Service
- Maintains responsibility for both the clinical and the distributive activities of the Pharmacy Department as necessary
- Assists with staffing as deemed necessary.
