Job Description
PURPOSE OF POSITION:
The Med to Bed program services our patients being discharged from the hospital. It helps to identify and address any medication access barriers to ensure that patients have their medications and are counseled on appropriate use before their discharge. The primary goals of this service are to help facilitate a smooth transition out of the hospital, promote discharge therapy adherence, and help prevent non-adherence related readmissions. A Meds to Beds Clinical Pharmacist will be responsible for the quality assurance of the pharmacy services to patients discharged from the Kings County Medical Center. This position is a part of the interdisciplinary team that includes clinical pharmacists, case management, pharmacy technicians, and medical providers
SUMMARY OF DUTIES AND RESPONSIBILITIES:
Minimum Qualifications
For Level I:
1. Possession of a valid NY State license to practice pharmacy.
For Level II: In addition to meeting the qualification requirements of the lower level:
1. Two years of post licensure experience; or
2. Completion of a post graduate specialty residency or fellowship program; or
3. Current Pharmacy Board Certification.
For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:
1. One year of post licensure supervisory experience or specialty clinical experience.

Job Description

SUMMARY OF DUTIES AND RESPONSIBILITIES:
- Focus on patients recently discharged who are at high risk for readmissions.
- Assists with the medication reconciliation process at discharge in coordination with the interdisciplinary care team to prevent any medication omissions, dosing errors, continuation of incorrect medications, therapeutic duplications, or clinically significant drug interactions.
- Collaborates with the inpatient care team and outpatient pharmacy team to help identify and address any medication access barriers to optimize medication regimen before discharge.
- Provides medication education and counseling to patients as part of Med to Bed program operations, to optimize patient's transition of care.
- Actively coordinate post-discharge follow-up related to medications for high-risk patients to aid in the reduction of readmission.
- Responsible for reviewing patient profiles for prescription order verification, appropriateness of medications, monitoring patient therapy, monitoring for drug safety and efficacy, and performing and documenting interventions when deemed appropriate
- Oversight of pharmacy technicians/interns and workflow
- Facilitates access to medications due to insurance coverage or prior authorizations
- Ensure medication errors and ADRs are reported and pathways developed to prevent future occurrences
- Maintains proficiency in decentralized pharmacy services and clinical pharmacy programs
- Support quality improvement projects and research initiatives
- Develop and conduct target drug programs and medication use evaluations as needed and report results to the Pharmacy and Therapeutics Committee.
- Monitor the use of expensive medications to ensure there is use consistent with approved criteria.
- Provide education to staff pharmacists as needed
- Maintains responsibility for both the clinical and the distributive activities of the Pharmacy Department as necessary
- Assists with staffing as deemed necessary
