The Home Care Registered Nurse is responsible for the assessment, development, implementation, and oversight of a home-based care plan for each assigned participant, incorporating both custodial home care and skilled home health needs.

SPECIFIC RESPONSIBILITIES:
  • Conducts comprehensive assessments and reassessments of participants' home care and home health needs, taking into consideration the participant's health conditions, functional levels, family support, and home environment.
  • Serves as a member of the participant's assigned Interdisciplinary Team (IDT) of healthcare professionals.
  • Utilizes assessment findings to collaborate with other IDT disciplines to develop an integrated, person-centered care plan, ensuring that all home-based care needs are addressed and coordinated with other services and supports.
  • Works in collaboration with the IDT to continuously monitor and adjust the IDT's Care Plan to ensure it remains appropriate and responsive to changing needs.
  • Monitors and evaluates the provision of home care and home health services through regular home visits and participant, family, and caregiver contact.
  • Initiates appropriate nursing procedures within scope and fulfills the requirements of a home health nurse under the IOA home health license and as directed by Providers.
  • Provides education and health care instructions to participants, family members, and/or home care staff and assumes a nursing case management role for assigned caseload.
  • Supports the home care staffing plan for training, on-boarding, competency testing, supervision, and evaluations. Provides field supervision and instruction for home care workers and family members in participants' homes.
  • Documents all care and services according to departmental, programmatic, and regulatory guidelines.
  • Participates in relevant committees and process improvement projects, such as Infection Control, Utilization Review, Quality Improvement, etc.
  • All other reasonably related tasks as assigned.

REQUIRED QUALIFICATIONS:
  • Graduate from an accredited school of nursing.
  • Unrestricted license as an RN in the State of California
  • Current BLS certification
  • At least two years of clinical nursing experience, at least one of which is in home health
  • At least one year of experience working with older adults


  • PREFERRED QUALIFICATIONS:
  • Public Health Nurse (PHN) certification
  • Geriatric nursing expertise and palliative care experience.
  • Bilingual English/Chinese (Cantonese or Mandarin) or English/Spanish


  • SKILLS:
  • Strong clinical skills in assessment, medication management, wound care, chronic disease management, and emergency response.
  • Ability to provide health education and teaching
  • Excellent communication and interpersonal skills
  • Strong technical skills and computer literacy
  • Strong critical thinking and effective problem-solving abilities
  • Knowledge of the community served. Ability to work within an interdisciplinary team with initiative, imagination, resourcefulness and flexibility.
  • Cultural competency and the interest and ability to work within a multilingual, multicultural environment.


  • PHYSICAL REQUIREMENTS:
  • Must be able to lift, transfer, push/pull, maneuver, and reposition 35lbs
  • Ability to reach, bend, walk, stand, and climb stairs
  • Manual dexterity for medical procedures, and able to operate medical devices and tools
  • Five finger manipulation and fine motor skills
  • Good vision to observe patient conditions, read medical instruments
  • Able to hear patients, family members and other healthcare professionals as well as alarms from medical devices


  • WORKING CONDITIONS:
  • May be exposed to extremes of heat and cold in all weather conditions. Must drive or take public transportation in various weather conditions on roads in varying degrees of repair.
  • May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances. May be exposed or occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations.
  • Travel required.


COMPENSATION
  • Range: $101,335 - 119,218


This rate is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, client needs, education and other factors.

This range does not include any additional equity, benefits, or other non-monetary compensation which may be included.
